Community Support Mentor

JOB HIGHLIGHTS

Responsibilities:
<ul>
<li>Ability to support individuals to live independently in their community</li>
<li>Work as a mentor and build relationships with individuals to assist them to connect to their community</li>
<li>Ability to support individuals with daily activities needed to live independently. This can include, but is not limited to cleaning, cooking, laundry, medication support, bill paying and personal care support</li>
<li>Ability to develop community relationships that fosters connections between the person supported and community organizations (volunteer or join a club for example)</li>
<li>Ability to make decisions and work independently</li>
<li>This position requires a company email address which requires an application to be downloaded on your personal cell phone for multi-factor authentication (MFA) to ensure cyber security.</li>
<li>Reliable vehicle (unless determined exempt with HR)</li>
</ul>
