Optimum Care Therapy

Essential Functions:<ul><li>Performs examinations and evaluations, and re-examinations , progress report of patients related to movement dysfunction with various diagnoses as applicable to the plan of care.</li><li>Establishes a Physical Therapy diagnosis based on the examination and evaluation, organized in clusters, syndromes, or categories.</li><li>Establishes a prognosis predicting the optimal level of improvement in function and amount of time needed to reach that level.</li><li>Establishes a plan of care, and functional goals (specific, measurable, attainable, realistic, timely SMART goals) that incorporate the patient&rsquo;s and/or family&rsquo;s goals.</li><li>Performs interventions which are purposeful and skilled, comprised of timely coordination of care, communication with parties involved in patient care, and patient/family education.</li><li>Completes accurate patient documentation that justifies skilled need, according to Program and professional standards.</li><li>Performs discharge planning and termination of formal intervention based on achievement of anticipated goals and expected outcomes, to facilitate a smooth transition to the next level of care.</li><li>Meets Program productivity standards and outcome expectations (participates in data collection and analysis).</li><li>Effectively directs and supervises support personnel (PT Assistant and PT Aide)</li></ul>Skills:<ul><li>Be able to perform at the established guidelines for Program specific requirements.</li><li>Bilingual (Spanish and English) is highly preferred or be able to communicate and professionally assess a a Spanish-speaking patient comfortably.</li><li>Knowledge of WebPT electronic documentation is a plus.</li><li>Competence in computers and computer applications (word-processing, google docs, excel).</li></ul>Minimum Position Requirements:Education: Require Coursework/Training - Graduate of an accredited (CAPTE) Physical Therapy professional program.Newly Grads are welcome!Licenses/Certifications:<ul><li>Valid, current California license.</li><li>American Heart BLS Healthcare Provider level.</li></ul>Preferred Position Qualifications:Education: Master's or Doctorate Degree.Job Types: Full-time, Part-timePay: $95,000.00 - $108,000.00 per yearBenefits:<ul><li>401(k)</li><li>401(k) matching</li><li>Continuing education credits</li><li>Dental insurance</li><li>Flexible schedule</li><li>Health insurance</li><li>Paid time off</li><li>Vision insurance</li></ul>Work Location: In person
