Options for Community Living, Inc. is looking for a Case Manager to join our Mental Health Licensed Program! An ideal candidate should meet the following requirements:
<ul>
<li>High School Diploma and 2 years of qualifying experience*, or a Bachelor's degree and 1 year of qualifying experience.</li>
<li>Excellent time management skills required.</li>
<li>Valid driver's license, safe driving record, valid auto insurance and access to a vehicle is required.</li>
</ul>
<ul><li>Qualifying experience: Verifiable full or part time case management or case work with persons with a history of mental illness, homelessness, or chemical dependence.</li></ul>

Salary Range: $40,040/year ($22.00/hour) - $50,960/year ($28.00/hour)

The above salary range represents Options for Community Living's good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. When determining a salary offer, several factors may be considered as applicable (e.g., years of relevant experience, education level, language skillset, credentials, professional licensure, budget, and internal equity).

Schedule Options: (35 hours/week)

<li>Monday/Wednesday/Friday, 9:00 AM - 5:00 PM; Tuesday/Thursday, 12:00 PM - 8:00 PM</li>
<li>Monday/Wednesday/Friday, 12:00 PM - 8:00 PM; Tuesday/Thursday, 9:00 AM - 5:00 PM</li>
<li>Monday/Tuesday/Wednesday, 9:00 AM - 7:00 PM; Thursday, 9:00 AM - 6:00 PM (4-day work week)</li>
</ul>
Location: In person; based out of our Ronkonkoma site with field visits required.

Pay Type: Non-exempt

Responsibilities:

The Licensed Program Case Manager provides supportive case management services to individuals with mental illness in the apartment treatment program. Monitors maintenance of sites and provides on-site visits to scattered housing locations.
<ul>
<li>Plan and evaluate service plans and monitor objectives. Write progress notes on service plan weekly.</li>
<li>Document services provided with appropriate notes.</li>
<li>Perform site visits daily.</li>
<li>Monitor maintenance of sites.</li>
<li>Oversee medication maintenance and assist residents in the development of self-medication skills as needed.</li>
<li>Provide information regarding community resources and orient residents to the area.</li>
<li>Assist in securing vocational assessment services, job training and education programs.</li>
<li>Provide advocacy services to gain appropriate entitlements and services.</li>
<li>Intervene in situations requiring immediate attention to ensure safety of residents.</li>
<li>Provide mediation, counseling, behavioral intervention, and crisis management services.</li>
<li>Train and assist residents in the activities of daily living including household maintenance.</li>
<li>Provide transportation for residents as needed.</li>
<li>Consult and coordinate with clients' family and providers as needed for coordinated service delivery.</li>
<li>May occasionally attend team meetings and discharge meetings.</li>
<li>Ensure household cleanliness is maintained to agency standards and assist as needed. Conduct fire drills at sites as needed.</li>
<li>Notify program supervisor/manager of any medical or psychiatric emergency.</li>
<li>Any additional relevant tasks as assigned by management.</li></ul><div>
