As a Primary Care Provider at PACE North, you will:<ul><li>
Work as part of a interdisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants</li><li>
Deliver care in multiple settings:
<ul><li>On-site clinic</li>
<li>Participant homes</li>
<li>Partner skilled nursing facilities</li>
</ul></li><li>Build strong relationships with participants and their families through education and support</li><li>
Practice in a flexible care model that adapts to participant needs rather than rigid systems</li></ul>
Requirements:

We&rsquo;re looking for a compassionate and collaborative healthcare professional with the following:<ul><li>
A passion for serving others with a positive, person-centered approach</li><li>
Commitment to interdisciplinary decision-making that prioritizes:

<li>Respect for self-determination</li>
</ul></li><li>Graduate of an accredited Nurse Practitioner or Physician Assistant program</li><li>
Current and valid Michigan license as a Nurse Practitioner or Physician Assistant</li><li>
Strong clinical leadership skills</li><li>
Excellent interpersonal and communication skills</li><li>
Effective problem-solving abilities</li><li>
A collaborative mindset with a focus on shared decision-making</li><li>
Experience in primary care or geriatric care (preferred)</li><li>
An inspirational, motivational, and creative approach aligned with the PACE model of care</li></ul></div>
