Focused on enabling individuals to remain safely at home, the organization offers a wide range of services tailored to each participant&rsquo;s unique requirements through home care, clinics, and community programs.Job Overview We are seeking an energetic and compassionate Registered Nurse (RN) to join our Home Care team. In this vital role, you will deliver high-quality nursing care directly in the homes of our participants, ensuring their health and safety while promoting independence. Your expertise will support a diverse caseload, including individuals with chronic conditions, memory care needs, and those requiring acute or palliative care. This position offers an exciting opportunity to make a meaningful difference in the lives of older adults by providing expert clinical assessments and personalized care.Duties<ul><li>Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation.</li><li>Develop and implement individualized care plans aligned with each participant&rsquo;s needs and goals.</li><li>Administer medications, injections, IV infusions, and perform wound suturing as needed.</li><li>Manage complex conditions such as dementia, Alzheimer's disease, chronic pain, and post-acute recovery.</li><li>Coordinate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner for accurate documentation.</li><li>Provide education on disease management, medication adherence, nutrition, and health coaching to patients and caregivers.</li><li>Perform specialized procedures including catheterization, ostomy care, tube feeding management, and sterile techniques for infection control.</li></ul>Requirements<ul><li>Valid RN license in the state of practice with current CPR/BLS certification.</li><li>Critical care experience in settings such as ICU or emergency medicine is highly preferred.</li><li>Experience working with individuals with developmental disabilities or in memory/cognitive impairment settings.</li><li>Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-9/10) is a plus.</li><li>Knowledge of infection control practices, aseptic techniques, and laboratory procedures.</li><li>Ability to perform physical examinations, venipuncture, phlebotomy, and specimen collection accurately.</li><li>Strong communication skills for triage assessments and patient education.</li><li>Experience in home &amp; community care environments; familiarity with long-term care or hospice is advantageous.
