<div>SUMMARY:
The Master&rsquo;s Social Worker (MSW) works independently to assess the needs of, coordinate, and deliver social services for participants and families/caregivers. The MSW is a member of the Interdisciplinary Team (IDT) and participates in the assessment, intervention, management, and review of social service needs. The MSW provides care through a variety of modalities, including but not limited to bio-psychosocial assessment, interventions that include cognitive behavioral techniques, psychoeducation, consultation, and care planning/case management.
SPECIFIC DUTIES AND FUNCTIONS:<ul><li>
The MSW is a member of the IDT and performs a comprehensive bio-psychosocial assessments at the time of enrollment (initial) and 180-day intervals thereafter. Acute assessments are completed on an as-needed basis and aim to answer specific referral questions. Utilizes measurement-based care.</li><li>
Develops and implements appropriate, individual treatment plans for both participants and caregivers, including but not limited to:<ul><li>
Counseling (utilizing Motivational Interviewing and CBT techniques)</li><li>
Education</li><li>
Coordination of services in collaboration with the care team</li><li>
Risk Assessment/Crisis Intervention</li></ul></li><li>
Directs case management Issues, including but not limited to:<ul><li>
Housing Issues, including identifying least restrictive setting in accordance with participant&rsquo;s level of income affordability</li><li>
Financial Issues</li><li>
Safety Issues including, but not limited to: symptoms of abuse, neglect or exploitation; ability to live independently; decision-making capacity; any other issues warranting involvement of Adult Protective Services (APS)</li><li>
Psychiatric: changes in mood, behavior, cognition/mental status; suicidal/homicidal ideation; substance use/misuse</li><li>
Caregiver Burden</li><li>
Identifies the need to transfer to a higher level of care.</li><li>
Assists in coordinating discharge planning activities and processes.</li><li>
Educates and counsels the participant and/or caregiver regarding care needs, options and other related problems.</li><li>
Advocates to others on behalf of the participant and demonstrates accountability in resolving participant concerns or issues.</li></ul></li><li>
Works collaboratively to develop and communicate initial impressions, plans of care, changes in care, and progress in a timely fashion.<ul><li>
Helps to develop and implement the plan of care with the IDT, participant, and/or caregiver.</li><li>
Completes timely and accurate documentation in the electronic health record (EHR).</li><li>
Communicates changes in participants with IDT and family members.</li></ul></li><li>
Evaluates participant outcomes and progress toward achieving the objectives and goals of the care plan and communicates this information among other members of the IDT.</li><li>
Performs other related duties, as assigned by leader(s).</li></ul>
KNOWLEDGE, SKILLS AND ABILITIES:<ul><li>
Master&rsquo;s Social Worker from an accredited school of social work.</li><li>
Licensed to practice in the State of Michigan. Limited License may be considered depending on experience and number of supervised hours.</li><li>
One year experience working as a clinical social worker, preferably in the field of geriatrics.</li><li>
Overall knowledge of case management skills, short term interventions, and principles of human behavior.</li><li>
Demonstrated knowledge and skill in evaluating, planning, implementing, and monitoring patient-centered approaches to care.</li><li>
Knowledge of and compliance with applicable local, state, and federal laws and regulations. Cultural competence and an ability to care for a diverse population.</li><li>
Ability to learn and use electronic health record system.</li><li>
Proficient in Microsoft Word, Excel, Outlook, PowerPoint.</li><li>
Positive, flexible, and solution-focused attitude with ability to collaborate effectively in a team setting.</li></ul></div>

Social Worker - Pontiac

<div>SUMMARY:
Under the management of the PACE SEMI&rsquo;s Community Living Manager and the coordination of the Lead CLA, the Community Living Associate (CLA) provides supportive services to the participants (residents); including but not limited to the community setting, during transport and on appointments, or in the day health center while under the supervision of the manager, registered nurse, or lead CLA. The CLA&rsquo;s responsibilities for each participant are specified in the CLA Care Plan. The following statements are intended to describe the essential elements, functions, and requirements of the position. The list should not be taken as an exhaustive list of all responsibilities, duties, and skills required of an individual assigned to this job.
SPECIFIC DUTIES AND FUNCTIONS:<ul><li>
The CLA plan of care is based on the initial and on-going assessments that are performed by the manager, assigned RNCM/clinical team, and lead CLA.</li><li>
The CLA assists in providing and/or maintaining optimal physical and emotional comforts to the participants (residents) living in the community setting while embracing the culture, mission, values, and the practicing Ten Principles at PACE SEMI.</li><li>
The CLA assists in creating a nurturing supportive environment while helping participants (residents) achieve maximum self-reliance and independence within the community living setting.</li><li>
The CLA performs the care or services that are outlined in the individual participant&rsquo;s care plan; any care guidelines that are pertinent to the participant&rsquo;s diagnosis are specified in the care plan.</li><li>
The CLA performs a variety of tasks and services for the participants (residents), including:<ul><li>
Personal care up to and including 2-person assist</li><li>
1:1 interaction in DHC and CL as outlined in the plan of care</li><li>
Assistance with activities</li><li>
Assistance with meal preparation / cooking/ feeding/other nutritional needs</li><li>
Assistance with toileting / managing incontinence</li><li>
Simple treatments</li><li>
Grocery shopping</li><li>
Laundry services</li><li>
Coordination of care with participant and the DHC based on care plan</li><li>
Engage with resident elevating health concerns and customer services issues or needs appropriately</li><li>
Homemaking services / chores/ housekeeping services/sanitizing equipment</li><li>
All other duties as required by manager and lead CLA, as assigned</li><li>
Rounds q2 hour after 7pm-7am.</li></ul></li><li>
The CLA is responsible for accurate timely documentation as required by the community living setting; including but not limited to the care and/or services provided during participant (resident) encounter(s). The CLA keeps the manager, lead CLA, and clinical team informed of the participant&rsquo;s progress.</li><li>
Immediately reports all customer service complaints, all deviations from set schedule and appointments to supervisor /manager or lead CLA.</li><li>
The CLA responds to emergency situations in accordance with established policies and procedures.</li><li>
Works only within established scope of practice.</li></ul>
KNOWLEDGE, SKILLS AND ABILITIES:<ul><li>
H.S. diploma or GED required</li><li>
Valid Michigan Driver's license</li><li>
Current CPR card required</li><li>
Certification is optional though preferred, but must have one (1) year experience with a frail or elderly population</li><li>
Experience must be in a long-term care, hospital, home care setting, or PACE setting</li><li>
Required completion of a basic nursing assistant training program. Must have received certificate of completion from a formal training program (Direct Care, Nurse Assistant, Medical Assistant, etc.) outside of PACE SEMI</li><li>
The CLA participates in annual, mandatory in-service training and screening, including but not limited to: infection control, TB testing, safety training, and BLS training</li><li>
The CLA must possess the ability to establish and maintain interpersonal and interdepartmental relationships</li><li>
The CLA shows respect for the physical, spiritual, and well-being of participants and co-workers</li><li>
The CLA must have the ability to problem solve and interpret instructions</li><li>
The CLA must be able to organize and maintain flexible scheduling to coordinate job responsibilities</li><li>

CLA competency is monitored and maintained in a variety of ways:<ul><li>
Initially, through a skills check-off performance evaluation of specific tasks upon hire and orientation to the program, and annually thereafter</li><li>
Job related competency in-services including a practicum demonstration and written assessment</li><li>

Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.</li></ul>
SUMMARY OF WORKING CONDITIONS:<ul><li>
Working conditions are variable. With exposure to communicable diseases, due to participant environment. There is also potential for exposure to chemicals such as cleaning solutions and medications</li><li>
The CLA is supervised regularly in the community setting, by the manager and RNCM by direct or indirect supervision. At this time the participant&rsquo;s satisfaction with the Care Plan and CLA is evaluated</li><li>
Driving is required within PACE SEMI catchment area</li><li>
The CLA must have reliable transportation available on a daily basis, a current valid driver&rsquo;s license, must maintain an acceptable driving record, and provide proof of current automobile insurance</li><li>
The CLA must have the ability to transfer and assist with moving patients; frequent walking, bending, and lifting of forty (40) pounds or more may be needed in the performance of duties</li><li>
The CLA may be required to change participant assignments based on the changing acuity level, participant needs, and participant census.</li></ul></div>

Nursing Assistant (Community Living)

<div>SUMMARY:
The Transportation Manager is responsible for oversight of the transportation depot(s) within area of responsibility. This includes management of the Transportation Coordinators, Transportation Lead Drivers, and Transportation Drivers. The Transportation Manager will oversee several logistical activities related to transporting our senior citizens to and from various appointments and activities, and regular, timely delivery of various items. This role requires a strong passion for internal and external customer service, and relationship-building, all related to providing the best-in-class, concierge-level care for our program participants within PACE Southeast Michigan.
SPECIFIC DUTIES AND FUNCTIONS:<ul><li>
Plan and manage the transportation of program participants to and from the Day Health Center</li><li>
Plan and manage the transportation of program participants to medical appointments within the Day Health Center</li><li>
Facilitate weekly staff huddles<ul><li>
Cultivate healthy bottom-up communication from team members to communicate appropriate information regarding participant tolerance while riding on PACE Southeast Michigan vehicles and/or other related concerns</li><li>
Cultivate healthy top-down communication from leadership to team members to communicate updates as necessary</li></ul></li><li>
Must participate as a member of the Interdisciplinary Team (IDT) daily and collaborate to ensure best-in-class care for program participants</li><li>
Participate in the monthly All Leadership Team meeting to provide updates on the department to the extended leadership team</li><li>
Direct orientation of new employees in the transportation department; ensure that new employees are educated on all transportation policies and procedures and receive the necessary experience to complete the Trainee Checklist within the first month of employment</li><li>
Maintain established departmental and system policies, procedures, and objectives, including but not limited to the following: Quality assurance program, Safety, Environmental and Infection control standards</li><li>
Schedule and facilitate focused monthly trainings with all transportation staff and hold staff members accountable to attend meetings and in-services</li><li>
Responsible for the human resources function within area of responsibility which includes but is not limited to the following: hiring, developing, training, and coaching of the transportation staff</li><li>
As needed, ensure all driver credentials are submitted timely to Human Resources. Credentials include but are not limited to: Valid, unrestricted, Michigan Commercial Driver's License (Class B, Passenger endorsement), Valid CPR Certification, and Valid DOT Medical Card</li><li>
Ensure that all staff always observes all traffic laws and regulations and reports any violations. Communicate necessary information regarding traffic violations or accidents/incidents to Safety, Fleet, and Training Coordinator</li><li>
Assist staff in reporting and recording participant feedback using Improve PACE Forms (IPF)<ul><li>
Root cause, resolve and follow up appropriately on transportation-related challenges reported by program participants and transportation staff within regulatory time frame</li></ul></li><li>
Proactively engage in process improvement to create new policies/processes and revise existing policies/processes to address transportation challenges.</li><li>
Ensure transportation staff completes any necessary documentation logs concerning vehicle pre-trip, post-trip, vehicle maintenance and safety maintenance<ul><li>
Maintain current pre-trip, post-trip, vehicle maintenance and safety maintenance logs as required by record retention schedule</li></ul></li><li>
Assure that all vehicles are kept in safe and satisfactory working order according to vehicle maintenance schedules</li><li>
Participate in and assist staff in completion of all necessary annual mandatory training and competency testing with regard to the performance of job duties</li><li>
In conjunction with the Director of Transportation, develop and monitor the budget related to the Transportation Department</li><li>
Develop and maintain transportation metrics as assigned</li><li>
Conduct semi-annual and annual performance reviews for all transportation staff</li><li>
Oversee and approve payroll time sheets bi-weekly</li><li>
Performs other related duties as necessary, assigned or requested by Director of Transportation</li></ul>
KNOWLEDGE, SKILLS AND ABILITIES:<ul><li>
High School diploma or GED required. Bachelor&rsquo;s degree in business management, supply chain, logistics or 3 years&rsquo; experience in related field preferred</li><li>
Three years&rsquo; work experience in related field and/or supervision/management of five or more staff</li><li>
Must demonstrate basic computer competencies; able to develop competency with new software as needed</li><li>
