Registered Nurse (RN ) Job Summary: 
Registered Nurse ( RN ) Major Duties and Critical Tasks: 
<ul>
<li>Works using the guidelines established by the Nurse Practice Act, facility Policies and Procedures, and sound nursing judgment.</li>
<li>Assesses, plans and evaluates nursing care delivered to patients/residents requiring long-term and/or rehabilitation care.</li>
<li>Delivers nursing care to patients/residents requiring long-term and/or rehabilitative care .</li>
<li>Implements the patient/resident plan of care and evaluates the patient/resident response.</li>
<li>Directs and supervises care provided by other nursing personnel.</li>
<li>Provides input in the formulation and evaluation of standards of care.</li>
<li>Maintains knowledge of necessary documentation requirements .</li>
<li>Maintains knowledge of equipment set-up, maintenance and use (i.e. monitors, infusion devices, drain devices, etc.).</li>
<li>Maintains confidentiality and compliance with patient/resident rights, relating to all patient/resident/personnel information.</li>
<li>Provides patient/resident/family/caregiver education as appropriate and/or directed.</li>
<li>Conducts self in a professional manner in compliance with unit and facility policies .</li>
<li>Works rotating shifts, holidays and weekends as scheduled.</li>
</ul>
Registered Nurse ( RN ) Required Education and Experience: 
<ul>
<li>Must be a graduate of a basic education program in professional nursing.</li>
<li>A minimum of one year medical/surgical nursing experience in an acute care (hospital) setting or long-term care setting (acute/critical care is preferred).</li>
<li>Must possess a current Registered Nurse ( RN ) license in state(s) of practice. Must have a reliable source of transportation.</li>
</ul>

Registered Nurse ( RN ) Supervisor: 
The Registered Nurse ( RN ) Supervisor Assists the Director of Nursing; plans, develops, organizes and coordinates day-to-day functions of the nursing department.
 
Registered Nurse ( RN ) Supervisor Major Duties and Critical Tasks:
 
<ul>
<li>Plans, develops, organizes, implements, and directs nursing services for residents on the floor assigned.</li>
<li>Ensures compliance with current applicable federal, state and local regulations and facility policies and procedures.</li>
<li>Reviews complaints and grievances made or filed by staff. Makes appropriate reports to the Director of Nursing as required or as necessary.</li>
<li>Ensures an adequate stock of medications, medical supplies, equipment, etc., is always maintained on the unit to adequately meet the needs of the resident and notifies appropriate personnel of needs.</li>
<li>Assists in the development of written preliminary and comprehensive assessments of the nursing needs of each resident.</li>
<li>Makes nursing/aide assignments as indicated.</li>
<li>Encourages staff to attend and participate in outside training programs and schedules times as appropriate.</li>
</ul>
Registered Nurse ( RN ) Supervisor Education and Experience: 
<ul>
<li>A nursing degree from an accredited college or university.</li>
<li>Minimum of two (2) years of experience providing direct care in long-term care, restorative or geriatric settings as an</li>
<li>Minimum one (1) year of experience as a nursing supervisor, preferably in a nursing home setting</li>
<li>Current CPR Certification.</li>
<li>Must also meet state requirements for relevant licensures or certifications and have no disciplinary action in effect against professional license.</li>

MDS Nurse Summary Description: The MDS Coordinator under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team.
 
MDS Nurse Functions and Responsibilities: 
<ul>
<li>Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.</li>
<li>Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments.</li>
<li>Communicates schedule to all other departments and families, as required.</li>
<li>Monitors clinical systems for changes in patient/resident condition.</li>
<li>Responds to patient/resident change in condition by coordinating patient/resident re-assessment and re-evaluation plan of care.</li>
<li>Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident.</li>
<li>Accurately completes portions of MDS and CAAs as assigned, in accordance. with policies, procedures, and guidelines.</li>
<li>Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process.</li>
<li>Coordinates and develops plan of care as required.</li>
<li>Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file.</li>
<li>Maintains Medicare Certifications /Re-Certifications</li>
<li>Participates in QAPI /QM process</li>
<li>Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.</li>
