What You Will Do 
 
<ul><li>Coordinate and complete required MDS assessments for residents. Make sure they are accurate and submitted on time to meet regulatory guidelines.</li><li>Check medical records and team documentation to ensure the information supports the MDS coding and reflects the resident&rsquo;s condition and care needs.</li></ul> 

What We Are Looking For 
 
<ul><li>Active and Clear LPN license</li><li>1-3 years of MDS experience in a healthcare setting</li></ul>

What You Will Do<ul><li> 
Assess new residents and develop care plan. Update therapy plans based on their recovery, safety needs and goals.</li><li>Guide residents in exercises, mobility training, strengthening, balance work and fine motor activities to help residents regain function after illness, injury or surgery</li><li>Document progress, challenges, and improvements in the medical record and communicate updates to the clinical team.</li></ul> 
What We Are Looking For<ul><li> 
Active and Clear PT license in the State of Florida</li><li>A caring attitude and strong teamwork skills</li><li>Skilled nursing or rehab experience is a plus but not required</li></ul>
