Qualifications:<ul><li>
Have at least 2-3 years' experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations</li><li>
Must have an active and unrestrictive Texas APRN license</li><li>
Must have DEA license or eligible to obtain it within 2 months of date of hire</li><li>
Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model</li><li>
Excellent documentation and patient relationship-building skills</li><li>
Proficient with EMR systems (eCW a plus!)</li></ul>
