</div><div></div><div><div>The Healthcare Center of Patriot Heights in San Antonio, TX is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a<b> full time inpatient and outpatient Hybrid</b><b> </b><b>Physical</b><b> </b><b>Therapist </b>for our in-house rehab program!</div>
<div></div><div><b>Key Responsibilities:</b></div>
 <div><ul><li>Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.</li>
 <li>Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.</li>
 <li>Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.</li>
 <li>Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.</li>
 <li>Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.</li>
</ul></div><div><b>Qualifications:</b></div>
<div>Physical Therapist license is required.</div>
<div><div><div>Open to Physical Therapists at all experience levels. New Grads are welcome!</div>
