The Retreat at Covington is seeking a Wellness Nurses to join their team!
The Wellness Nurse reports directly to the Wellness Director.
Shift Details
11pm-7am; Rotating Schedule Including Weekends
PURPOSE
The Wellness Nurse is responsible for monitoring the health and wellbeing of our residents and provides leadership support and assistance with resident care and services.
Responsibilities include but are not limited to the supervision of overnight Resident Care Associates (RCA), communication with resident support services and families and maintaining clinical quality assurance while leading, demonstrating and supporting all the elements of wellness which are core components of Phoenix programs and services.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Resident Care<ul><li>
 Identifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) process.</li><li>
 Ensures proper follow through and documentation for residents overnight.</li><li>
 Assist the Wellness Director with supervising overnight Resident Care Associates (RCA) and chart organization.</li><li>
 Informs the Wellness Director and/or Executive Director of any changes in residents' condition.</li><li>
 Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates</li><li>
 Assist with direct resident care as needed</li></ul>
EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION<ul><li>
 Must be a Licensed Practical Nurse or Registered Nurse</li><li>
 A minimum of one year of experience working with elders or disabled individuals, preferred</li><li>
 Must have demonstrated Leadership capabilities</li><li>
 Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred</li><li>
 Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times</li></ul>

 Demonstrated knowledge of state regulations. Proficient in using state audit compliance tools</li><li>
 Is proficient in time management skills and adherence to deadlines</li><li>
 Has exceptional grammar and documentation skills</li><li>
 Understands basic concepts of risk management</li></ul>

Wellness Nurse

<div>Description:

The Phoenix at Lake Joy is seeking an RN/LPN to join their team as a Wellness Director!
The Wellness Director is responsible for providing overall leadership and management of the health and well-being of the residents within the community. Responsibilities include but are not limited to all clinical programs, quality assurance and regulatory compliance; assisting in the training and on-boarding of clinical and care coordinator team members. The Wellness Director is also responsible for assisting and partnering with the Regional Director of Resident Care to provide Medication Care Manager training (in states where allowed.
Requirements:

EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION
Nurse | RN | LPN
Must be a Registered Nurse or Licensed Practical Nurse with an Associate or Bachelor&rsquo;s degree
Must have a minimum of 2 years department management experience working with elders or disabled individuals
Must have demonstrated Leadership capabilities
Multi-department experience strongly recommended
Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing
