An Internal Medical Group has an opportunity for a Physician to work in Skilled Nursing Facilities in the New London area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The position would be full time working 40 hours per week.

Summary

Under indirect supervision, provides and manages health care for a specified patient population. 

Essential Functions
<ul>
<li>Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.</li>
<li>Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.</li>
<li>Analyze patients medical history, medication allergies, physical condition and examination to determine best care</li>
<li>Diagnose medical condition</li>
<li>Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.</li>
<li>Refers patients to specialists or to internal medicine as appropriate.</li>
<li>Directs and coordinates the patient care activities of nursing and support staff as required.</li>
<li>Respect and promote patient rights and privacy.</li>
<li>Responding appropriately to emergency situations.</li>
<li>Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.</li>
<li>Participate in specified health promotion, education and/or prevention programs.</li>
<li>Participates in population health activities as directed by the practice.</li>
<li>Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards</li></ul> 
 
Skills and Knowledge
<ul>
<li>Knowledge of legal and ethical standards for the delivery of medical care.</li>
<li>Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.</li>
<li>Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.</li>
<li>Demonstrated skill in interviewing and patient coaching.</li>
<li>EHR documentation as it pertains to patients and populations of patients.</li>
</ul>
Education and Experience
<ul>
<li>Medical Doctor with 3 to 5 years of direct patient experience which may include residency.</li>
<li>Medical License, Board certification, Federal DEA certification</li>
<li>Need to be Board eligible or Board Certified</li>
