 At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they&rsquo;re most comfortable&mdash;creating a seamless, convenient, and personalized process for seniors and their caretakers.
 
 
 
 
 
 The Opportunity
 
 
 
 The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH in our Reno Clinic location as well as patients residing in assisted living communities, and residential care homes.
 
 
 This care includes-
 
 
 <ul>
 <li>
 Obtain history and physical on new patients
 </li>
 <li>
 Order/interpret diagnostic tests
 </li>
 <li>
 Diagnose and treat chronic conditions and episodic illness
 </li>
 <li>
 Perform Advance Care Planning
 </li>
 <li>
 Accurate and timely documentation in the EHR
 </li>
 <li>
 Coordinates with other members of the care team, both internal and external partners
 
 
 </li>
 </ul>
 
 Responsibilities
 
 
 
 In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines. The assessments may be done in the clinic or residential setting. This service includes:
 
 
 <ul>
 <li>
 Conducts Health Assessment, including past medical history, review of systems, medication reconciliation, and physical exam
 </li>
 <li>
 Review and discuss the need to close care gaps and quality measures
 </li>
 <li>
 Perform/review standardized screening exams
 </li>
 <li>
 Identify all existing medical conditions
 </li>
 <li>
 Accurately and timely documentation of assessment and conditions to include quality MEAT Statements
 </li>
 <li>
 Identifies patients in need of chronic care management
 </li>
 <li>
 Identifies patients in need of pharmacy assistance
 </li>
 <li>
 Document Elements of Personal Prevention Plan and health issues to follow up with PCP
 
 
 </li>
 </ul>
 The Provider is also responsible for ensuring a safe and smooth transition of patients following an acute or skilled stay or ED visit. The care included is-
 
 
 <ul>
 <li>
 In-home post-acute visit including history and physical and medication reconciliation
 </li>
 <li>
 Short-term management of episodic illness related to acute or skilled admission
 </li>
 <li>
 Care coordination with team members for the implementation of a safe discharge plan
 </li>
 <li>
 Identify barriers/obstacles to safe, self-management in the community and work with care team to overcome or identify alternatives
 
 
 </li>
 </ul>
 
 Qualifications
 
 
 
 <ul>
 <li>
 Current Nevada NP license, credentialed with Medicare, Medicaid and health plans PPH is actively contracted with (Senior Care Plus, Prominence, OPTUM,) or has the ability to be enrolled with any health plans.
 </li>
 <li>
 Minimum two years experience in specialty care area (Geriatrics) in private physician office, clinic setting or community-based program preferred.
 </li>
 <li>
 Demonstrates clinical competence including strong communications skills and medical knowledge.
 </li>
 <li>
 Remains current in licensure according to state regulation.
 </li>
 <li>
 Positively represents the practice by communicating to all visitors, patients and staff in an enthusiastic, friendly and professional manner.
 </li>
 <li>
 Provides a positive environment and experience with patients
 </li>
 <li>
 Actively seeks out methods to support the practice, staff, patients and physicians
 </li>
 <li>
 Participates in Continuing Medical Education as required by their governing board

 Love working remotely? Hold a California RN license? Pine Park Health is looking for a Registered Nurse who loves caring for older adults and thrives in a remote, tech-enabled environment. This is a fully remote role &mdash; you can live anywhere, as long as you hold an active, unrestricted California RN license. As an RN at Pine Park, you're not a support function. You're the hub of the care team. You'll work alongside Primary Care Providers, Medical Directors, and Medical Assistants to deliver coordinated, high-quality primary care to residents of senior living communities across California, Nevada, and Arizona &mdash; all from your home office. If you're a clinically sharp RN who communicates well, takes ownership, and wants to do meaningful work without a commute, this role was built for you.
 
 
 
 What You'll Do
 
 
 
 <ul>
 <li>
 Collaborate with a dedicated team, supporting a substantial patient panel alongside Primary Care Providers (PCPs).
 </li>
 <li>
 Triage inbound communications, including calls, faxes, email and instant messages from teammates, patients and partners. You will close out what you can, escalate to the PCPs when necessary, and route sub-clinical tasks to our Medical Assistants.
 </li>
 <li>
 Act as a support system and escalation point for LVN/LPNs within the team, enhancing our team's efficiency and effectiveness.
 </li>
 <li>
 Engage in comprehensive chart preparation and provide crucial patient education on care plan adjustments.
 </li>
 <li>
 Oversee post-appointment processes, including diagnostic orders, referrals, and medication orders.
 </li>
 <li>
 Participate in daily huddles, offering guidance and fostering teamwork among your PCPs and MA teammates.
 </li>
 <li>
 Conduct virtual patient assessments, developing personalized care plans and monitoring health outcomes.
 </li>
 <li>
 Serve as a pivotal communication link among patients, PCPs, specialists, and other healthcare professionals.
 </li>
 <li>
 Promote patient health through education on preventive care and self-management strategies consistent with the patient&rsquo;s care plan and wishes.
 </li>
 <li>
 Participate in crisis intervention and palliative care discussions, focusing on patient comfort and quality of life.
 </li>
 <li>
 Engage in ongoing quality improvement initiatives and contribute to research efforts aimed at enhancing geriatric care.

 What We're Looking For:

 A current unrestricted multi-state RN license with at least 2 years of clinical experience, preferably in geriatrics or related fields.
