What you&rsquo;ll do</b></div><div>
The Residential Reentry Center Resident Monitor I is responsible for guiding, assisting and supporting residents in their transition back into community life by carrying out the day-to-day operations of the facility. The Resident Monitor I is responsible for ensuring program and facility safety, monitoring resident movement, verifying resident employment, and supporting structured cognitive and behavioral-based programming designed to promote risk reduction and successful community reintegration. This position maintains a continuous state of alertness and awareness in order to ensure control and security of the facility and residents.</div><div><br>
</div><div>Primary/essential duties and responsibilities are but not limited to:
</div><ul><li>Monitors and controls entrance to a secure facility
</li><li>Completes intake, program orientation, and discharge procedures for all residents upon arrival and at exit</li><li>
Performs scheduled and random urinalysis testing, inspections, shakedowns, counts, or breath tests</li><li>
Conducts pat-searches of residents and inspects belongings; conducts contraband control and confiscation</li><li>
Supervises and monitors resident movement, activity, and rule compliance</li><li>
Organizes and supervises resident work details to assist in maintaining facility cleanliness
</li><li>Assesses and responds appropriately to resident behaviors, attitudes, and dispositions</li><li>
Documents disciplinary action for rule violations in accordance with contracting agency requirements</li><li>
Stores, monitors, and dispenses resident medications following prescribed protocol</li><li>
Welcomes visitors with professionalism and courtesy; verifies proper identification for clearance and entry
</li></ul><ul><li>Conducts physical grounds and facility safety checks</li><li>
Assesses, evaluates and reports internal and external safety, sanitation, and security concerns</li><li>
Patrols building to observe residents and activities for safety and compliance</li><li>
Conducts and logs facility counts ensuring accuracy of all accountability documentation</li><li>
Ensures compliance with all health, hygiene, safety and maintenance requirements</li><li>
Conducts scheduled and impromptu cleanliness checks of the facility and residents&rsquo; personal belonging
</li><li>Takes appropriate corrective action to ensure work is performed safely and without injury to self or others</li><li>
Operates and monitors video and GPS surveillance technologies</li><li>
Ensures all equipment necessary for each shift is operational and documents as necessary

</li><li>Receives, gives, logs, and archives accurate shift reports addressing incidents, medical problems, discipline, new arrivals, and special instructions</li><li>
Ensures accurate data entry into a community corrections software platform to include resident photos</li><li>
Completes file audits to include medication control, contraband control, and urinalysis tracking
</li><li>Monitors delivery, storage, preparation and service of meals; cooks and serves food in absence of kitchen staff</li><li>
Maintains confidentiality and ensures sensitive information is shared only on a &ldquo;need-to-know&rdquo; basis</li><li>
Completes all required trainings
</li><li>Assists with coordination and administrative tasks related to inspections, program reviews, and internal/external accreditation audits

WHAT YOU'LL DO</b></div><div>
The Health Coordinator supports Pioneer Human Services&rsquo; mission by coordinating and monitoring client physical and behavioral health services within residential treatment facilities. This role assists in ensuring continuity of care by facilitating communication between clients, nursing staff (where present), medical and behavioral health providers, pharmacy services, and program staff.</div><div></div><div><br>
The Health Coordinator supports the intake process, coordinates internal and external medical and behavioral health appointments (including telehealth), assists with medication monitoring and documentation accuracy in the electronic medication administration record (eMAR/MAR), and gathers and communicates relevant health information to appropriate clinical staff. At sites with licensed nurses, the Health Coordinator works under the direction of nursing staff to support care coordination and implementation of provider orders. At sites without nurses, the Health Coordinator works under the direction of Health &amp; Resource Coordination (HRCU) Manager.</div><div></div><div><br>
In addition to care coordination, the Health Coordinator provides direct client support within a structured residential environment, including; engaging clients in the milieu, observing and monitoring client changes in physical and in behavioral health status, reinforcing treatment plans and health education, assisting with access to healthcare services, and responding to urgent health concerns by notifying appropriate clinical staff or emergency services as needed.</div><div></div><div><br>
This role requires strong organization, attention to detail, and the ability to effectively support clients with complex medical, behavioral health, and substance use needs in a structured residential environment.</div><div></div><div><b><br>
PRIMARY/ESSENTIAL DUTIES AND RESPONSIBILITIES:</b></div><div><b>
Client Service:</b></div><ul><li>
Utilize program-approved screening tools and established procedures to identify and document client risks, needs, and barriers while supporting client and facility safety.</li><li>
Utilize Motivational Interviewing (MI) and other engagement strategies to support client participation and skill development.</li><li>
Interpret and explain program policies and goals to clients during orientation and throughout treatment as needed.</li><li>
Support client education by conducting health education activities and reinforcing self-care skills appropriate to the client&rsquo;s stage of recovery.</li><li>
Where applicable by site or unit, observe and document client self-administration of medications in accordance with program policies and procedures.</li><li>
Where medication responsibilities are assigned to nursing staff, promptly communicate medication-related concerns, discrepancies, or client requests to the licensed nurse.</li><li>
When applicable to the role at a specific site, review MAR documentation for completeness at shift change and escalate discrepancies according to policy.</li><li>
Maintain a safe and healthy environment which includes maintaining, cleaning, and disposing of equipment and supplies according to policy and procedures (e.g., cleaning medication rooms, disinfecting vital signs equipment, maintaining health services spaces).</li><li>
Where applicable, assist with medication storage, ordering, and returns in accordance with pharmacy services policies and site procedures.</li><li>
Assist clients in meeting health and safety needs in accordance with each individual&rsquo;s stage of recovery and program policies.</li><li>
Provide basic first aid and serve as the first point of contact for emergency response when needed, notifying licensed nursing staff, providers, or emergency services as appropriate.</li><li>
Follow provider orders and support implementation of care plans in accordance with program policy and the scope of the Health Coordinator role.</li><li>
Gather and relay client-reported health information, including health and medication history, vital signs, symptoms, diagnoses, and other relevant concerns to nursing staff or providers as appropriate.</li><li>
At sites with licensed nurses, assist Registered Nurses (RN) or Licensed Practical Nurses (LPN) with intake processes or coordination activities as directed.</li></ul><div></div><div><b><br>
Coordination of Services:</b></div><ul><li>
Support coordination of client physical and behavioral healthcare services during assigned shifts, including telehealth visits, clinic appointments, and outside provider services.</li><li>
Coordinate and document client intake and discharge processes after hours, including financial documentation and secured belongings as required.</li><li>
Coordinate internal and external services and referrals based on provider recommendations or treatment planning needs.</li><li>
Support service navigation by assisting clients in accessing health services and communicating needs to clinical staff.</li></ul><div></div><div><b><br>
File and Document Management:</b></div><ul><li>
Maintain accurate, timely, and complete documentation of services in the electronic health record.</li><li>
Ensure required notifications are completed during assigned shifts.</li><li>
Complete eligibility assessments and follow-up assessments in designated systems.</li><li>
Ensure client information is accurate and updated from intake through discharge to support continuity of care.</li><li>
Record routine case notes and service contacts documenting client progress and services provided.</li><li>
Review reports with the care team to identify service gaps and coordinate appropriate follow-up support.</li></ul><div></div><div><br>

QUALIFICATIONS:</b><br>

High School Diploma or Equivalent</li><li>
One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations</li><li>
Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain and maintain active registration within 90 days of hire in accordance with WAC 246-810-015; RCW 18.19.210</li><li>
First Aid/CPR certification within 90 days of hire</li><li>
Proficient level of written/verbal communication, and efficiency with independent task management and ability to show attention to detail.</li><li>
Basic computer skills and the ability to learn</li><li>

The Case Manager is the primary contact between residents and program goals. This position is responsible for executing assessments, documenting results and progress in the client management system, actively collaborating with community resources, communicating routinely with all participants on their case load, providing direct services and coordinating referrals to external agencies for services. The Case Manager assists participants in moderating the effects of incarceration, substance abuse, financial distress, family needs, housing, and mental health.

Assess individual risk, needs and barriers using validated tools and techniques, assure client and facility safety</li></ul><ul><li>
Maintain professional boundaries with clients</li><li>
Deescalate clients in need of assistance</li><li>
Develop and maintain working knowledge of community resources</li><li>
Assists first responders as needed (i.e. police and medical personnel)</li></ul><ul><li>
Complete and document a resident needs assessment upon intake
</li><li>Collaborate with Community Recourses to meet resident needs</li><li>
Facilitate relevant group sessions to meet resident needs
</li><li>Work with Program staff to assess resident behavior and needs</li><li>
Coordinate resident entry into needed community programs, such as detox, residential treatment, housing, etc.
</li><li>Routinely contact each resident based on the appropriate level of service</li><li>
Conduct individual goal-setting and case-planning sessions with each resident</li><li>
Utilize motivational Interviewing (MI) practices
</li><li>Utilize results of needs assessment and other information to assist with completion of short-term and long-term goals</li><li>
Review and monitor case plans with client, documenting progress
</li><li>Document all assessments and progress in efforts to outcomes database including: progress notes, needs assessment, barrier tracking at scheduled intervals, direct services provides, external referrals coordinated, and other relevant information.</li></ul><div></div><div><b><br>

Bachelor's degree in social work, psychology, criminal justice or related field</li><li>
One year of experience working with marginalized populations
</li><li>Valid Washington State driver's license.</li></ul><ul><li>
Employees who operate their own or PHS vehicles on PHS business, must have a safe driving record as defined by PHS.</li><li>
Employees who operate their own vehicle(s) on PHS business must carry auto liability insurance that complies with the requirements of PHS.</li><li>
