Job Responsibilities</b></div></div><div><ul><li>Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT)</li><li>Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT</li><li>Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions</li><li>Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities</li><li>Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments</li><li>Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies</li><li>Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist</li><li>Complete accurate and timely electronic documentation in compliance with Medicare home health regulations</li><li>Follow infection control standards and home health safety protocols</li><li>Participate in QAPI and agency performance improvement initiatives</li></ul></div><div><div>
