Responsibilities<ul><li>
Evaluate residents and develop individualized treatment plans</li><li>
Provide skilled physical therapy services to improve mobility, strength, and independence</li><li>
Supervise PTAs and collaborate with the interdisciplinary care team</li><li>
Participate in care planning and discharge preparation</li><li>
Educate residents, families, and caregivers</li><li>
Maintain accurate and timely clinical documentation</li></ul>
Qualifications<ul><li>
Current California Physical Therapist (PT) license</li><li>
Strong clinical and communication skills</li><li>
Commitment to resident-centered care</li><li>
Ability to work effectively within a multidisciplinary team</li></ul>
