Working closely with the Chief Medical Officer, provider liaisons, clinical teams, care navigators, and patients, the APP will review clinical documentation, complete CMS-required attestations, and support timely, high-quality dementia care. This role helps ensure that patients and caregivers have access to the services and support they qualify for.Key ResponsibilitiesClinical Responsibilities<ul><li>Review and sign off on documentation from care navigators and nurses in a timely manner.</li><li>Enter relevant charges into the electronic medical record (Athena) for services provided.</li><li>Complete provider attestations as required for the dementia care program(s).</li><li>Provide oversight to care navigators and nurses supporting program beneficiaries.</li><li>Respond to escalations from clinical teams and care navigators as needed.</li><li>When appropriate, evaluate and diagnose patients with dementia when extenuating circumstances prevent assessment through their existing care team.</li><li>Communicate clearly and frequently with patients, caregivers, PocketRN staff, and partners.</li><li>Follow escalation protocols for urgent scenarios and ensure timely communication with relevant parties.</li><li>Maintain accurate, timely, and thorough documentation of patient data and clinical decisions.</li><li>Maintain strict adherence to HIPAA and privacy standards in a virtual care environment.</li></ul>Telehealth &amp; Administrative Responsibilities<ul><li>Maintain an active, unencumbered APRN license in all states where care is delivered.</li><li>Review and sign off on completed comprehensive assessments, medication reconciliations, and required attestations within 7 days of the patient visit, when serving as the billing provider. This applies to patient visits conducted by nursing staff that requires provider review for completion and billing purposes.</li><li>Participate in patient, caregiver, and staff education and coaching when requested.</li><li>Conduct remote calls and video visits on PocketRN, including urgent escalation visits when needed. Appropriate documentation must be completed in 24 hours.</li><li>Complete all required training to stay current with PocketRN clinical protocols and workflows.</li><li>Complete all required clinical competency assessments and orientation checkoffs within 30 days of the start date, demonstrating readiness to independently manage provider visits, and meet organizational clinical standards.</li><li>Assist with the creation of clinical content and training materials as requested.</li><li>Perform administrative tasks as needed to support program operations.</li><li>Cooperate with billing, documentation, credentialing, payor enrollment, and compliance requirements.</li><li>Participate in quality assurance, peer review, and performance improvement initiatives.</li><li>Prepare accurate and timely electronic medical record entries.</li><li>Notify PocketRN within 72 hours of any event that may affect licensure, credentialing, or professional standing.</li></ul>QualificationsRequired<ul><li>Dementia Proficient Provider, meeting at least one CMS criterion:</li><li>At least 25% of patient panel within the past 5 years comprised of adults with cognitive impairment (including dementia), or</li><li>At least 25% of patient panel within the past 5 years aged 65 or older, or</li><li>Specialty in neurology, psychiatry, geriatrics, geriatric psychiatry, behavioral neurology, or geriatric neurology.</li><li>Education: Graduate of an accredited school of nursing.</li><li>Experience: Minimum 2 years of direct patient care as an Advanced Practice Provider.</li><li>Licensure: Current APRN license(s); New Mexico or Alaska</li><li>Communication: Strong verbal and written communication skills; proficiency with asynchronous communication in a remote environment.</li><li>Professionalism: Courteous, empathetic, and patient-centered communication.</li><li>Technical Skills: Proficiency with electronic health records (Athena preferred), Google Workspace, and telehealth tools.</li><li>Home Setup:</li><li>Private, quiet location for HIPAA-sensitive calls</li><li>Reliable computer (no Chromebooks)</li><li>Internet speed of at least 50 Mbps</li></ul>Preferred<ul><li>Experience working in virtual care or startup environments.</li><li>Experience supporting dementia or geriatric populations.</li></ul>Core Competencies<ul><li>Patient-Centered Care &ndash; Honors patient values, needs, and preferences.</li><li>Adaptability &ndash; Able to learn new systems quickly and adjust to changing workflows.</li><li>Self-Motivation &ndash; Works effectively in an independent, remote environment.</li><li>Mission-Driven Teamwork &ndash; Thrives in a collaborative, compassionate, innovative healthcare team.</li></ul>Benefits<ul><li>Fully remote work environment.</li><li>Extensive opportunities for professional growth, mentorship, and career advancement.</li></ul>Apply using this link https://jobs.apploi.com/view/1706915?utm_source=false&amp;utm_medium=&amp;utm_campaign=trumanPay: $45.00 - $65.00 per hourEducation:<ul><li>Master's (Required)</li></ul>Experience:<ul><li>direct patient care as an Advanced Practice Provider: 2 years (Required)</li></ul>License/Certification:<ul><li>New Mexico or Alaska CNP license (Required)</li></ul>Work Location: Remote
