Key Responsibilities:

<li>Virtual Visit Facilitation
<ul><li>Initiate outbound calls to patients at scheduled appointment times to prepare and connect them to their provider (video or telephone visits).</li>
<li>Ensure patients are ready for the visit, including confirmation of availability, technology readiness, and required documentation.</li>
<li>Coordinate real-time handoff between patient and provider to support on-time visit start.</li>
</ul></li><li>Appointment Reminders &amp; Outreach
<ul><li>Conduct appointment reminder calls, same-day confirmations, and follow-up outreach to reduce no-shows.</li>
<li>Communicate schedule changes, delays, or cancellations to patients and providers as needed.</li>
<li>Document all outreach attempts and patient interactions according to organizational standards.</li>
</ul></li><li>Provider Schedule Support
<ul><li>Monitor provider schedules throughout the day and proactively address gaps, late arrivals, or connectivity issues.</li>
<li>Serve as the first point of contact for visit-related operational issues during clinic hours.</li>

<ul><li>Accurately document visit status, outreach attempts, and outcomes in the designated systems (e.g., EHR, scheduling platform, call system).</li>
<li>Maintain up-to-date appointment notes and flags related to visit readiness or barriers.</li>
<li>Adhere to HIPAA and organizational confidentiality policies at all times.</li>

<ul><li>Collaborate closely with providers, medical assistants, nurses, and front office staff to ensure smooth visit flow.</li>

Required Qualifications:

Position Title: Care Navigator 
 
Location: Brooklyn, NY 11218 
 
Schedule: Full Time - Monday-Friday 9 am-5 pm 
Job Summary: As a Care Navigator, you will guide patients through the enrollment and onboarding process for RPM, CCM, and CHI programs. Your role is crucial in helping patients&mdash;particularly those in underserved communities&mdash;access and benefit from these services. You will coordinate patient care, facilitate communication between healthcare providers and patients, verify insurance eligibility, and maintain accurate patient records. By ensuring timely interventions and follow-ups, you will contribute to improved patient outcomes and reduced hospital visits. 
Key Responsibilities:

<li>Assist patients in enrolling in RPM, CCM, and CHI programs, providing clear guidance throughout the process. 
</li>
</ul><ul><li>Schedule and manage patient appointments, including enrollments, escalations, follow-ups, and healthcare visits such as M11q and DOH-type appointments.

</ul><ul><li>Address patient inquiries and escalate concerns related to RPM, CCM, and CHI services to appropriate healthcare professionals.

</ul><ul><li>Conduct insurance eligibility checks via platforms such as e-PACES, verifying coverage for RPM, CCM, and CHI services.

</ul><ul><li>Enhance provider efficiency by performing pre-appointment cross-checks in ChronicCareIQ (CCIQ) and HHA Exchange, ensuring patient charts are verified, updated, or entered accurately.

</ul><ul><li>Maintain effective, professional communication with patients and team members through phone, email, and other correspondence, fostering a supportive and welcoming patient experience.

</ul><ul><li>Oversee device ordering and setup for RPM, ensuring patients receive and understand how to use their devices for effective monitoring.</li></ul> 


Requirements:

<li>High school diploma required; additional education or certification in Medical Administration is preferred.

</ul><ul><li>Previous experience in sales, ideally in a healthcare setting.

</ul><ul><li>Proficiency in using RPM and CCM platforms (e.g. ChronicCareIQ) and EMR systems (e.g., AdvancedMD).

About the Role

We&rsquo;re looking for a dedicated and compassionate Registered Dietitian Nutritionist (RDN) licensed in New York State to join our expanding telehealth team. This is a unique opportunity to provide impactful, one-on-one medical nutrition therapy (MNT) and counseling to patients across New York &mdash; all from the comfort of your home.

Key Responsibilities
