Company Overview Portage Path Behavioral Health is a dedicated non-profit community mental health center committed to supporting individuals with low to moderate incomes in their journey toward recovery from mental illness and substance use challenges.Job Overview We are seeking a highly skilled and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for our Primary Care Team. This role involves delivering high-quality patient assessments, managing acute and chronic conditions, and supporting our mission to provide effective, affordable mental health and medical services within a community setting. This position is contract, seeking 8 to 16 hours of weekly coverage (Monday - Friday, no holidays and no weekends).Key Responsibilities<ul><li>Provide comprehensive primary care services including assessment, diagnosis, and treatment of acute and chronic conditions</li><li>Conduct physical exams and obtain medical histories</li><li>Order and interpret laboratory and diagnostic tests</li><li>Prescribe medications and treatments in accordance with state and federal regulations</li><li>Provide preventive care including wellness exams, screenings, and immunizations</li><li>Manage chronic diseases such as diabetes, hypertension, and asthma</li><li>Educate patients on disease prevention, lifestyle modifications, and medication adherence</li><li>Collaborate with physicians, behavioral health providers, dentists, and care coordinators in a team-based care model</li><li>Document care accurately in the electronic health record (EHR)</li><li>Participate in quality improvement initiatives and population health efforts</li></ul>Qualifications<ul><li>Graduate of an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program</li><li>Current state licensure and national certification</li><li>Current DEA license or eligibility</li><li>Current BLS certification</li><li>Strong commitment to serving medically underserved populations</li><li>Experience in family medicine, community health, or primary care (preferred)</li></ul>Work Location: In person
