 Primary Care Nurse
 
 to join our Primary Care team at HMP Millsike, a
 
 category C
 
 
 Prison located in
 
 Full Sutton, East Yorkshire.
 
 
 
 
 HMP Millsike is a newly built prison with an operational capacity of 1500 places. This is a rare opportunity to be part of a new healthcare team from the inception of a brand new prison site.
 
 
 
 
 In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.
 
 
 
 Hours
 
 
 
 Weekdays: 07:00 - 19:15
 
 
 Weekends: 08:15 - 18:00
 
 
 Rolling 5 week shift pattern, working 1 weekend in 5
 
 
 
 Salary
 
 
 
 This position is full time, you&rsquo;ll receive an annual salary up to
 
 &pound;41,659
 
 
 
 per annum - depending on experience.

 What you'll be doing
 
 
 As a Primary Care Nurse your responsibilities will vary. You will
 
 
 <ul>
 <li>
 Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
 </li>
 <li>
 Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
 </li>
 <li>
 Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
 </li>
 <li>
 Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.

 NMC Registered nurse &ndash; Adult
 </li>
 <li>
 Evidence of Continued Professional Development
 </li>
 <li>
 Understanding of the complexities of multi-disciplinary and multi-agency working
 </li>
 <li>
 A non-judgmental and compassionate approach
 </li>
 <li>
 Excellent communication skills
 
 
 </li>
 </ul>

 We have an exciting opportunity for an Advanced Clinical Practitioner to join our Primary Care Team at HMP Altcourse, a category B prison located in the Fazakerley area of Liverpool.
 
 
 
 Salary
 
 
 
 This position is full time, you&rsquo;ll receive a salary of &pound;66,792 per annum.
 
 
 
 Hours
 
 
 
 This is a full time role working 37.5 hours per week.
 
 
 Monday- Friday
 
 
 8am-4pm

 What you'll be doing
 
 
 As a Advanced Clinical Practitioner, your responsibilities will vary. You will
 
 
 <ul>
 <li>
 Demonstrate judgement in assessing and managing wide ranging and complex patient problems
 </li>
 <li>
 Provide clinical leadership for the mental health team, This multi-disciplinary team is made up of a variety of registered and non-registered professionals including; mental health, and substance misuse practitioners
 </li>
 <li>
 You will act as a role model ensuring professional appearance and maintaining professional standards at all times
 </li>
 <li>
 Supporting senior clinicians on site you will be part of the daily leadership team, providing clinical input for all patients
 </li>
 <li>
 As a clinical expert in your field, you will work with the team to ensure; patient safety, the management of complex cases and ensure the most up to date treatment methodology is applied to patient care.

 Qualifications Required
 
 
 
 <ul>
 <li>
 Registered Nurse &ndash; Adult Mental Health with current NMC or Registered Paramedic with current HCPC
 </li>
 <li>
 Independent/supplementary prescriber qualification
 </li>
 <li>
 MSc in Advanced Practice
 </li>
 <li>
 Experience working autonomously as an advanced clinical practitioner following best practice guidance
 
 
 </li>
 </ul>
 Although not essential ideal applicants would have experience in a hospital, community or prison environment.
