Position Summary:</b></p><p>
The Pediatrician provides comprehensive primary care for infants, children, and adolescents within Nuvia Medical&rsquo;s value&#8209;based care model. The physician is accountable for outcomes across a defined patient panel under a per&#8209;member&#8209;per&#8209;month (PMPM) arrangement&mdash;proactively managing preventive care, chronic conditions, and utilization to improve quality and reduce avoidable emergency department visits and hospitalizations. Care is delivered in collaboration with care coordination staff and the Quality Assurance team, as well as other Nuvia Medical stakeholders, to close gaps in care, meet HEDIS and Continuity&#8209;of&#8209;Care (COC) measures, and support optimal patient and family experience. The majority of patients are Medicaid members (with smaller Medicare and fee&#8209;for&#8209;service populations), and this role requires a commitment to serving underserved communities with culturally competent, family&#8209;centered care.</p><p><b>
Essential Duties and Responsibilities:</b></p><div><ul><li>
Provide full&#8209;spectrum outpatient pediatric care, including well&#8209;child care, immunizations, acute illness management, and chronic disease management (e.g., asthma, diabetes, obesity).</li><li>
Conduct age&#8209;appropriate screenings and assessments (e.g., development, behavioral/mental health, vision/hearing, BMI counseling) with timely follow&#8209;up.</li><li>
Manage a defined pediatric patient panel in a PMPM/value&#8209;based environment, with accountability for quality, utilization, and patient/family satisfaction outcomes.</li><li>
Identify, monitor, and close pediatric gaps in care by collaborating with care coordination staff and the Quality Assurance team.</li><li>
Ensure compliance with relevant HEDIS pediatric measures and payer quality programs (e.g., well&#8209;child visits, immunization status, appropriate testing and medication use).</li><li>
Promote appropriate resource utilization to minimize avoidable ED use and inpatient admissions/readmissions through early intervention and care navigation.</li><li>
Develop individualized care plans for medically complex or high&#8209;risk pediatric patients, coordinating with specialists, schools, and community resources as needed.</li><li>
Participate in population health initiatives to improve preventive screening rates, medication adherence, and chronic condition control across the pediatric panel.</li><li>
Review clinical dashboards and quality reports; implement targeted interventions to improve outcomes and support medical loss ratio (MLR) and shared&#8209;savings goals.</li><li>
Educate and engage patients and families through anticipatory guidance, shared decision&#8209;making, and clear follow&#8209;up plans.</li><li>
Document all encounters accurately and promptly in the electronic health record (EHR) in accordance with organizational and regulatory standards.</li><li>
Maintain current knowledge of pediatric clinical guidelines, immunization schedules, and evidence&#8209;based practices in value&#8209;based care.</li><li>

The Family Medicine Physician provides comprehensive primary care services to a defined panel of patients within Nuvia Medical&rsquo;s value-based care delivery model. The physician is responsible for achieving optimal health outcomes, improving quality performance, and reducing unnecessary utilization through proactive management of patient panels. Care is delivered in collaboration with care coordination staff and the Quality Assurance team, as well as other Nuvia Medical stakeholders, to close gaps in care, meet HEDIS and Continuity-of-Care (COC) measures, and reduce avoidable ER visits and hospitalizations.<br>

 This role is critical to Nuvia&rsquo;s mission of providing compassionate, accessible, and high-quality healthcare to underserved populations. The majority of patients are Medicaid members, with a smaller portion covered under Medicare and fee-for-service plans.
