 <b>RESPONSIBILITIES:</b></p><p>
As a Registered Nurse with Primary Care Solutions, you will enjoy the privilege of providing healthcare of the highest quality to America&rsquo;s Veterans as part of a Patient Aligned Care Team (PACT) and will have an active role in developing and implementing a patient care plan.</p><ul><li>
Office hours are Mon - Fri 8:00am to 4:30pm.</li><li>
This is a Full-Time position onsite &amp; remote (2 weeks onsite/2 weeks remote)</li><li>
Assist practitioners with patient care and practice specialized nursing skills in patient assessment</li><li>
Triage urgent/emergent same-day patients for the team. Identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes</li><li>
Provide support by assisting our patients in navigating and coordinating their care</li><li>
Participate in a daily team huddle</li><li>
Answer and returns patient telephone calls, messages and alerts</li><li>
Provide patient and family education focused on self-management, prevention, and wellness</li><li>
Monitor discharge list and contact patients for follow up</li><li>
Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met</li><li>
Promote achievement of clinical goals and metrics</li></ul><p>
 <b>JOB REQUIREMENTS:</b></p><ul><li>
1 year of Primary Care and/or Chronic Disease Management experience</li><li>
Current, active, and unrestricted state Nursing License</li><li>
Current BLS for providers from the American Heart Association (to be renewed annually)</li><li>
Strong computer skills, including EMR experience.</li></ul><p>

As a Licensed Practical Nurse, you will be responsible for providing a friendly, welcoming, and confidential environment for our Veterans who have served our country valiantly. We are seeking healthcare professionals who are driven and committed to ensuring the health and well-being of our nation's Veterans.</p><p><b><br>
</b></p><ul><li>Office hours are Mon - Fri 8am to 4:30pm.</li><li>
This is a telehealth and back office position.</li><li><b>
This is a Full - Time position on site 5 days per week.</b></li><li>
Interviews patients, measures vital signs, and records information in patient&rsquo;s electronic medical record</li><li>
Obtain medical histories from patients; verify patient information</li><li>
Prepare, assist, and accurately complete all forms</li><li>
Administer medications and operate diagnostic equipment</li><li>
Assist providers during examinations and treatment</li><li>
Take vital signs and perform all ancillary tests appropriate to skill level, certifications, and state regulations</li><li>
Perform ancillary testing and tasks as ordered by the Center Medical Director (PFT, EKG, injections, audiograms, blood draws, etc.) and as certified</li><li>
Collect lab Specimens from patients and submit received results to medical providers</li><li>
Complete all clinical reminders</li><li>
Answer and return patient telephone calls, messages, and alerts</li></ul><p></p><p><b><br>
REQUIREMENTS:</b></p><ul><li>
High School Diploma/GED</li><li>
Graduate of an accredited certificate program</li><li>
Current and active state Nursing License</li><li>
One year of continuous experience in clinical operations preferred/<b>New Grads welcome!</b></li><li>
Knowledge of computer systems to include the MS Office Suite</li><li>
Current Basic Life Support for providers certification from The American Heart Association</li></ul><p></p><p><b><br>
