This successful hospice company is a premier provider of end-of-life care offering a generous salary, an industry leading benefits package and opportunities for advancement.The RN Case Manager is the member of the interdisciplinary team who is the pivotal person in identifying the physical, psychological, social and spiritual needs of the patient and family. He/She initiates appropriate intervention and support for the patient and family upon admission and provides a continuously appropriate, comprehensive and responsive plan of care.<ul><li>This position is Monday through Friday, 8am to 5pm.</li><li>Coordinates the patient&rsquo;s and families care and services including those of the team, social worker, chaplain, home health aide, volunteer and physician.</li><li>Responds necessary information to the Team Manager.</li><li>Monitors the re-certification process.</li><li>Participates in Interdisciplinary Team Meetings as indicated.</li><li>Documentation is complete, accurate and timely.</li><li>Ensures and documents appropriate use of the medications, durable medical equipment and supplies.</li><li>Educates patients, families and other caregivers.</li><li>Provides skilled nursing as determined by the interdisciplinary plan of care.</li><li>Updates care plan regularly and integrates plan of care with any new changes in patient condition.</li><li>Appropriately utilizes the skills and resources of contract personnel such as PT, OT, Dietary, Speech and Pharmacy.</li><li>Performs on site evaluations of Certified Home Health Aides (CHHA) and volunteers.</li><li>Participates in the on-call rotation, weekends and holiday schedules if situation arises.</li><li>Integrates Quality Improvement results into the established plan of care.</li><li>Participates in the orientation of new team members and / or act as a preceptor to nursing students as assigned by the Team Manager.</li><li>Attends regularly scheduled in-services and educational conferences.</li><li>Performs other duties as assigned.</li></ul>Requirements<ul><li>Resides in Southern California area.</li><li>Current/valid Registered Nurse licensed in the State of California.</li><li>Registered Nurse shall have a minimum of (a) one (1) year experience as a professional nurse within the last (3) years and holds a current CA RN license. Previous experience in hospice or home health preferred.</li><li>Knowledge of the principles and practices of primary medical and nursing care.</li><li>Demonstrates the ability to use good listening and communication skills.</li><li>Able to multi-task effectively.</li><li>Capacity to relate and work as a member of an IDT.</li><li>BLS required within 60 days of hire.</li><li>Current California driver's license and proof of insurance.</li></ul>Job Type: Full-timePay: $44.23 - $54.19 per hourBenefits:<ul><li>401(k) matching</li><li>Dental insurance</li><li>Flexible spending account</li><li>Health insurance</li><li>Life insurance</li><li>Paid time off</li><li>Tuition reimbursement</li><li>Vision insurance</li></ul>Work Location: On the road
