 About The Position
 
 
 
 This is entry level skilled craft position whereby the incumbent operates non- commercial passenger vehicles and ancillary equipment (wheelchair lift, two-way radio) for the purpose of transporting customers with special needs (senior citizens, person with disabilities) to assigned destinations. Para-transit routes are primarily followed; however fixed routes may be assigned if necessary. The incumbent is evaluated based on the safe and efficient operation of vehicles as relates to the transport of passengers and the quality of services provided to customers. Work is performed outdoors in varying weather conditions with exposure to the elements. Incumbent must be physically able to repetitively lift and carry at least 50lbs on a regular basis and must wear and maintain a uniform and\safety equipment. Work is performed under the supervision of a Transit Service Coordinator (TSC) within well-established policies, procedures, and guidelines. Schedules may include weekend routes.
 
 
 
 About The Agency
 
 
 
 The Department of Public Works and Transportation (DPW&amp;T) is responsible for nearly 2,000 miles of County-maintained roadways that range from rural to urban classifications. The transportation infrastructure includes 900 bridges, as well as shoulders, sidewalks, curbs/gutters, stormwater management facilities, driveway aprons and nearly 3,000 acres of grassy area -- all of which are maintained by staff of the Department. DPW&amp;T&rsquo;s workforce keeps the County&rsquo;s various services moving by performing a myriad of activities which include removing snow and ice from County-maintained roadways, upgrading traffic signals, installing streetlights, clearing fallen trees, unclogging drainage inlets and maintaining stormwater management facilities, as well as performing roadway repairs ranging from potholes and sidewalk trip hazards to bridge repairs and resurfacing projects and more.
 
 
 Examples of Work

 Safely operates non-CDL vehicles in passenger service for Call-a-Bus and/or PGC Link while adhering to department policies and federal, local, and state laws.
 </li>
 <li>
 Utilizes mobile transit technology to follow schedule and notify passengers of trip progression.
 </li>
 <li>
 Maintains communication by radio with Dispatch and Transit Service Coordinators to report service interruptions.
 </li>
 <li>
 Greet passengers and provide general customer service by responding to inquiries and providing general transit information.
 </li>
 <li>
 Assist passengers boarding/disembarking with wheelchairs or other mobility aids to include their proper securement.
 </li>
 <li>
 Ensures cleanliness of vehicle exterior/interior and fuels vehicle after each shift.
 </li>
 <li>
 Conducts routine inspections of assigned vehicle before/after use according to safety and operating procedures and reports any defects accordingly.
 </li>
 <li>
 Participate as an Essential employee in the Department's Snow and Ice Control Program and other emergency or special activities.

 Qualifications

 Graduation from high school or a GED equivalency

 One (1) year of transit operation experience with direct customer service experience

 About The Position
 
 
 
 The Prince George's County Department of Family Services is seeking qualified applicants to fill multiple Limited Term Grant Funded (LTGF) Community Developer I positions, grade G-18, within its Aging and Disability Services Division. This is entry level professional work responsible for advocating, counseling, assessing, developing, and providing community liaison/social service assistance to individuals interested in the Medicaid Home and Community Based Services Programs (Community Options, Community First choice, Community Personal Assistance Services, Money Follows the Person) and long-term care options. The incumbent may be required to work occasionally on some evenings or weekends. Work is performed and evaluated under close supervision of the Program Manager.

 The Department of Family Services improves the quality of life and overall well-being of the communities by providing information, assistance and referrals, as well as promoting and developing high quality, innovative programs that educate, empower, respect choice and preserve dignity. The department is responsible for providing assistance to some of the County&rsquo;s most vulnerable citizens &ndash; children, families, victims of domestic violence, people with intellectual and developmental disabilities, veterans, and seniors. The department is comprised of three divisions that serve the aging and disabled, victims of domestic violence, as well as children, youth and families in need of support and resources.
 
 
 Examples of Work

 Provide professional social/work/case management services to individuals seeking enrollment in one of the Medicaid Home and Community Based Programs (Community Options, Community First Choice, Community Personal Assistance Services, Money Follows the Person).
 </li>

 Support Planners will provide program education on the Medicaid programs eligibility, and application/enrollment process. Provide options counseling to nursing home residents and their guardians/family members interested in learning about services and support in the community specifically related to Medicaid programs.
 </li>
 <li>
 Provide ongoing case management services to include evaluation of the individuals&rsquo; need/medical assessment, assistance with development of care plans utilizing a person-centered approach.
 </li>
 <li>
 Conducting quarterly visits to monitor and ensure that the services are being provided in accordance with the plan of service, program policies and procedures.
 </li>
 <li>
 Make appropriate referrals for community resources based on client need, identifying Waiver and non-Waiver services.
 </li>
 <li>
 Provide assistance to nursing home residents interested in transitioning to a community setting.
 </li>
 <li>
 Assist with identifying affordable housing options and addressing housing related barriers.
 </li>
 <li>
 Establish and maintain participants' information and case records in the Program database and ensure that all information is current and updated.

 Preparing case notes and other reports are required.
 </li>

 Collaborate with local Health Department, Dept of Social Services, Maryland Dept of Health and other community organizations concerning the operation of the Program and its participants.
