In this vital role, you will provide comprehensive primary care services to adult patients, within a highly vulnerable population focusing on health equity, disease prevention, health promotion, and management of acute and chronic conditions. Your expertise will ensure patients receive personalized, evidence-based care in a compassionate environment.
