Care Transition Coordinator Job Description

Reports to Director of Marketing and/or Region Manager

Job Summary: Care Transition Coordinators serve as the primary liaison between the company and referral sources. Responsible for developing, maintaining and servicing community relations in several settings. Serves as a resource to provide education and information on company&rsquo;s programs and services to patients, caregivers, physicians, case managers, discharge planners, social workers and others. CTC is responsible for obtaining patient referrals within the assigned territory for the company.

In lieu of degree, five or more years of experience in Business to Business and/or industry selling experience.</li><li>Desired: Bachelor&rsquo;s Degree in Business, Nursing, Marketing or Healthcare Related Field. Formal sales training also preferred.</li></ul></div>

Primary Job Functions:
<ul><li>Manage Sales Performance</li><li>Monthly Home Health and/or Hospice budgets are met on a consistent basis.</li><li>Review of territory on a regular basis and discussed with manager in order to optimize market potential.</li><li>Sales meetings are held to work with operations in order to achieve goals of the company. CTC is expected to be prepared, organized and engaged in all subject matters.</li></ul>
<ul><li>Territory Management</li><li>Assigned territory is effectively and efficiently targeted to include communication and education of home health and hospice services while utilizing available resources.</li><li>Developing and implementing weekly and monthly call plans with optimal territory productivity.</li><li>Strategic planning for the territory.</li><li>Identify new referrals sources and maximize the opportunity to grow business within the entity.</li><li>Efficiently cover assigned territory on a weekly, monthly, and quarterly basis.</li><li>Knowledge and understanding to be maintained on all systems used to track referrals and referral sources so that data can be obtained to discuss trends at sales meetings.</li><li>Territory specific daily and weekly call quotas are met to optimize territory.</li><li>Assignment report will be accurately maintained.</li></ul>

<ul><li>Administrative Duties</li><li>Prompt attendance and engaged in all meetings.</li><li>All assignment given by manager are completed on time.</li><li>Sales calls are document appropriately and by deadline of Monday at 8am.</li><li>Expenses logged and reporting done by deadline</li><li>Follow up on all pending referrals</li><li>Timesheets and mileage are completed by 9am Monday morning.</li></ul>

Working Conditions: 
Travel by car to physician offices, hospitals, community centers, and events with exposure to all extremes of weather. Environments may be unpredictable and not under control of the Agency.

<ul><li>Must be able to perform limited to full range heavy work requiring sitting, standing, walking and driving.</li><li>Must possess vision, hearing and physical dexterity necessary to maintain a driver&rsquo;s license and operate an automobile.</li><li>Must be able to climb stairs, access and operate elevators. Must be able to maneuver on all types of terrain.</li><li>Must possess vision, hearing, feeling, physical strength and dexterity necessary to perform all duties of a Care Transition Coordinator.</li><li>Must be able to communicate effectively both orally and in writing. Licensed Physical Therapy Assistant (LPTA)

JOB SUMMARY:

A Licensed Physical Therapy Assistant (LPTA) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Physical Therapist (PT).

QUALIFICATIONS:

1. Graduate from a two-year college level program approved by the American Physical Therapy Association.

2. Currently licensed, registered or certified in the state(s) in which practicing.

3. Two (2) years&rsquo; experience, preferred.

4. Acceptance of philosophy and goals of Agency.

5. Ability to exercise initiative and independent judgment.

RESPONSIBILITIES:

1. Understands and adheres to established Agency policies and procedures.

2. Provides physician prescribed physical therapy under a plan of care established by the PT.

3. Improves or minimizes residual physical disabilities of the patient.

4. Returns the individual to optimum and productive level within the patient&rsquo;s capabilities.

5. Participates with all other health care personnel in patient care planning.

6. Performs all skilled procedures as ordered by physician and according to the plan of care established by the PT.

7. Consults with PT regarding change in treatment.

8. Instructs patients and family members in home programs and activities of daily living.

9. Participates in inservice programs and presents inservice programs as assigned.

10. Participates in QAPI activities as assigned.

11. Attends all patient care conferences as scheduled.

12. Prepares medical records, progress notes and updates care plans for each patient visit in a timely manner as per Agency policy.

13. Performs services planned, delegated and supervised by the PT.

14. Assists in preparing clinical and progress notes.

15. Participates in educating the patient and family.

WORKING ENVIRONMENT:

Works indoors in Agency office and patient homes and travels to/from patient homes.

JOB RELATIONSHIP:

1. Supervised by: Registered Physical Therapist/Director of Clinical Services/Nursing Supervisor

Ability to perform the following tasks if necessary:

Key Responsibilities
