Summary Job Description:

The Medical Assistant performs a wide variety of duties and responsibilities in a manner that emphasizes quality of care and customer service. The MA works collaboratively with all Clinical services staff in support of direct patient services, exhibiting flexibility and a positive attitude. Patients are the key priority in this position, requiring the MA to serve as a point of contact in an environment that promotes patient comfort and trust. The position must exemplify the core values and mission of the organization, exercising discretion, diplomacy, and tact in patient/staff interactions. The MA communicates to the Lead MA immediate concerns regarding patient interaction. The MA reports to the Health Center Administrator.

Essential Duties and Responsibilities:

 Specific tasks include:
<ul>
<li>Triage patients, including Blood Pressure, Weight, Height, Pulse, and Temperature</li>
<li>Administer Injections, Immunizations, and Venipuncture according to orders in the EHR for adults and children</li>
<li>Collection of Lab Specimens</li>
<li>EKG if needed</li>
<li>Responsible for reporting to other sites as needed</li>
<li>Responsible for all front Desk Functions in a Primary Care setting</li>
<li>Schedule Appointments</li>
<li>Schedule specialty and ancillary appointments</li>
<li>Answer Phones</li>
<li>Responsible for patient registration data entry</li>
<li>Verify insurance</li>
<li>Daily Batching of encounter forms</li>
<li>Follow up for reports from specialists</li>
<li>Assist Providers and Clinical staff as needed</li>
<li>Assist patients as needed</li>
<li>Review patients' charts and encounter forms for completion</li>
<li>Scan Reports to the EHR and distribute</li>
<li>Ensure that prescription refills and voicemails are retrieved daily</li>
<li>Extensive medical terminology</li>
<li>Report Incidents as they occur.</li>
<li>Complete assigned training as required</li>
<li>Utilizes CPR skills if necessary</li>
<li>Translate using Spanish (if bilingual)</li>
<li>Other duties as assigned</li>
</ul>

<li>Comply with Department of Public Health (DPH), The Joint Commission, and other accreditation and regulatory agencies' standards.</li>
<li>Adhere to all PHMC Policies and Procedures.</li>
<li>Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures as described in the annual education module.</li>
<li>Maintain MDS sheets according to federal and state regulatory agencies</li>
<li>Keeps all required Certifications current</li>
<li>Completes all Continuing Education requirements according to PHMC University</li>

EXPERIENCE:

One to two years of experience in a clinical laboratory/medical office environment.

Bilingual (Spanish Speaking).

<li>Diploma from Accredited Medical Assistant School</li>
<li>Medical Assistant Certification</li>
<li>CPR Certification</li>
</ul>
PHYSICAL DEMANDS:

Position requires standing 2/3 of the time, walking 2/3 of the time, requires sitting under 1/3 of the time, use of hands and/or fingers to handle or feel 2/3 of the time, ability to reach with hands and arms, under and/or over 1/3 of the time, stoop, kneel, crouch or crawl under 1/3 of the time, talk or hear over 2/3 of the time. Position requires lifting to 10 lbs. up to 1/3 of the time.

 WORK ENVIRONMENT:

Moderate noise (examples: business office with computers and printers, light traffic) and exposure to blood-borne pathogens that will require the use of personal protective equipment.

JOB OVERVIEW:

Lead Medical Case Manager oversees care coordination and provides a full range of support for clients diagnosed with HIV/AIDS at the Care Clinic. The Lead Medical Case Manager reports to the HIV Program Manager.

Responsibilities:
<ul>
<li>Maintain a caseload of people living with HIV/AIDS.</li>
<li>Provide oversight of Medical Case Managers (MCMs) and conduct patient chart reviews.</li>
<li>Conduct MCMs monthly supervisions.</li>
<li>Complete a biopsychosocial assessment and service care goals with each client at the time of intake. Monitor and update service care goals and assessments quarterly in coordination with the interdisciplinary team.</li>
<li>Collaborate with an interdisciplinary team in the evaluation of patient needs and treatment progress.</li>
<li>Maintain contact with clients based on program need, client need, and service care goals.</li>
<li>Documentation of adherence to medical appointments and medical care plan, including HIV medication adherence.</li>
<li>Provide trauma-informed therapeutic support in collaboration with an interdisciplinary team, including crises.</li>
<li>Identify appropriate resources and external support systems to address client needs, including advocating and negotiating linkages to resources.</li>
<li>Available to provide home visits and accompany clients to medical appointments and outside treatment facilities as necessary.</li>
<li>Maintain accurate and timely documentation of all client contacts (using D.A.P. format), forms, and applications in EHR software.</li>
<li>Coordinates and documents Psychosocial Support Groups as required by the Division of HIV Health (DHH).</li>
<li>Assist Program Manager with Quality Improvement Projects as directed by DHH.</li>
<li>Perform analysis and reporting of MCM data as required by DHH.</li>
<li>Provide real-time data support to MCM staff.</li>
<li>Support the assessment of impact and outcomes to improve clinical care and meet reporting obligations.</li>
<li>Contribute as a team member and share in the responsibilities required to maintain operations and serve the mission of PHMC. This includes, but is not limited to, obtaining training, participating in events, flexibility in scheduling, covering the commitments of coworkers when they are unable, and other duties as assigned.</li>
<li>Perform other duties as assigned.</li>

<li>Bachelor&rsquo;s Degree required. Degree must be in Social Work, Human Services, Sociology, Psychology, Nursing, Public Health, or a related field from an accredited school. Other degrees may be considered if the candidate has Medical Case Management experience servicing people living with HIV/AIDS</li></ul>
