Puget Sound Home Health &amp; Hospice</b> is seeking a detail-oriented and collaborative <b>Palliative Medical Assistant </b>to join our team.</p><p></p><p><b>
This pivotal role for community-based palliative care/chronic care management (CCM) </b>provides administrative support to a care team that serves patients with serious, long-term illnesses. <b>This role involves working closely with patients and families, community partners, ARNP and other nursing staff, and other office support staff</b> to help coordinate care and improve overall quality of life.</p><p><b>

Qualifications</b></p><ul><li><p>
Graduate of an accredited Medical Assistant program; <b>current MA certification required</b> (AAMA, AMT, NHA).</p></li><li><p>
Current Basic Life Support (BLS) certification.</p></li><li><p>
Experience in palliative, hospice, or chronic care preferred.</p></li><li><p>
Strong communication, organization, and problem-solving skills; empathetic and calm under pressure.</p></li><li><p>
Proficiency with Electronic Health Records (EHR) and virtual health technologies</p></li><li><p>
**<i>Experience with Home Care Home Base is strongly preferred</i></p></li></ul><p></p><p><b>
Key Responsibilities:</b></p><p><b>
Administrative Support</b></p><ul><li><p>
Maintain accurate and timely documentation in the electronic health record (EHR), ensuring patient information and care coordination activities are properly recorded.</p></li><li><p>
Conduct outreach to patients with chronic conditions via phone, email, or approved virtual platforms to support follow-ups, care plan engagement, and appointment reminders.</p></li><li><p>
Serve as a communication hub between patients, families, providers, and community partners to ensure timely coordination of care and information sharing.</p></li><li><p>
Support provider and care team workflows by assisting with scheduling, calendar coordination, and tracking follow-up tasks or referrals.</p></li><li><p>
Answer and route incoming calls, assist with patient inquiries, and help maintain a professional and responsive front office environment.</p></li><li><p>
Assist with scheduling appointments and managing patient calendars to support efficient clinic operations.</p></li><li><p>
Monitor office and clinical supply levels, coordinate ordering, and maintain organized inventory records.</p></li></ul><p></p><p>
