What You'll Do:</b></h3>
<ul><li><b>Provide Direct Patient Care</b>: Administer treatments and medications as prescribed, develop individualized care plans, and offer compassionate guidance to patients and families.</li>
<li><b>Make a Real Difference</b>: Complete comprehensive assessments, evaluate patient conditions, and ensure care plans are aligned with evolving needs to ensure comfort and dignity at every stage.</li>
<li><b>Collaborate with an Expert Team</b>: Work closely with physicians, social workers, and other healthcare professionals to ensure optimal patient outcomes and provide holistic, patient-centered care.</li>
<li><b>Coordinate Care</b>: Lead efforts as a case manager for your assigned patients, ensuring seamless, high-quality care across all aspects of the patient's journey.</li>
<li><b>Teach &amp; Empower Families</b>: Provide education on self-care techniques, symptom management, and how to ensure patients are as comfortable and independent as possible.</li>

<ul><li><b>Registered Nurse (RN)</b> with a valid license in your state.</li>
<li><b>Experience in Hospice or Home Health Care</b>: Minimum of 1 year of professional nursing experience; hospice experience is required.</li>
<li><b>Compassionate &amp; Empathetic</b>: A heart for helping others, especially those facing end-of-life challenges.</li>
<li><b>Strong Communication Skills</b>: Ability to coordinate care with physicians, patients, and families, and clearly document clinical notes.</li>
<li><b>BSN Preferred</b>: Experience or education in the aged, chronically ill, restorative, or psychiatric nursing preferred.</li>

<ul><li>Graduate from an accredited nursing school (BSN preferred).</li>
<li>Valid RN license in the practice state.</li>
<li>Active BLS certification.</li>
<li>A licensed driver with an insured automobile.</li>
