POSITION SUMMARY: 
Plan, organize and direct the overall operation of overall services in SCALF in accordance with applicable regulations, policies, and guidelines to foster the highest degree of quality care.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:

<ul>
<li>Develop, review, and revise standards, guidelines, forms and job descriptions</li>
<li>Provide and/or promote staff educational programs, monitor attendance and compliance with mandatory training as required</li>
<li>Participate in regularly scheduled management meetings</li>
<li>Coordinate the completion of the admission, discharge, and evaluation materials and process for all residents. Ensure appropriate documentation including functional assessments.</li>
<li>Ensure that all resident rights and responsibilities are protected and enforced</li>
<li>Establish and maintain a close liaison with residents and families</li>
<li>Promote an environment that allows residents to function as independently as possible</li>
<li>Collaborate with the Medical Director and other professional service providers</li>
<li>Coordinate with appropriate department directors and supervisors for services provided to the residents</li>
<li>Maintain all documentation required by state agencies</li>
<li>Organize and promote educational opportunities for residents and families</li>
<li>Comply with state and federal requirements and regulations</li>
<li>Maintain current knowledge for scope of practice for self and support staff</li>
<li>Seek opportunities for personal and professional growth, related to clinical standards of practice, gerontology, rehabilitation, organization, management, regulatory agencies, infection control, and interpersonal skills</li>
<li>Monitor usage of supplies and equipment and plan for long-range equipment replacement</li>
<li>Review, investigate and resolve customer grievances in cooperation with the Administrator</li>
<li>Develop and coordinate the QA and QI process</li>
<li>Participate in presentations to appropriate community organizations and individuals to meet community established occupancy goals. Promotes a positive image</li>
<li>Maintain and dispose of discontinued controlled substances per state pharmacy regulations</li>
<li>Any other duties as assigned by Executive Director or Supervisor</li>
</ul>
SUPERVISORY RESPONSIBILITIES: 
<ul>
<li>Recruit, train, and schedule SCALF nursing and direct care staff</li>
<li>Supervise all members of the Nursing department with assistance of ED including coordinating and approving time off, conducting performance evaluations, counseling, and directing and managing the workflow for the department</li>
<li>Supervise staff to provide quality services in a caring environment, which strives to preserve the residents’ independence and self-respect.</li>
<li>Conduct regular staff meetings to exchange information and address issues affecting resident care</li>
<li>Promote job satisfaction</li>

Unit Coordinator (RN)

POSITION SUMMARY: 
Responsible for managing the operation of the nursing units in the facility. Assists the Director of Nursing with administrative duties and the supervision of nursing staff.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES: 
The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
 
<ul>
<li>Acts as liaison between nursing units, admissions and the DON.</li>
<li>Monitor resident performance and re-evaluating resident performance or lack of performance.</li>
<li>Attend meetings and committees to address resident care issues as needed.</li>
<li>Monitor weekly and monthly weights</li>
<li>Monitors and evaluates nursing staff and makes recommendations for training or work modification to the DON.</li>
<li>Monitor meals and breaks to assure compliant staff.</li>
<li>Monitor meals and temperature of meal trays.</li>
<li>Performs personnel management functions such as establishing personnel qualification requirements, drafting procedure manuals, initiating in-service programs, and installing record and reporting systems.</li>
<li>Monitor noise level and satisfaction of residents with care/or dissatisfaction.</li>
<li>Performs studies to evaluate effectiveness of nursing services in relation to their objectives and cost.</li>
<li>Monitor and audit charts weekly for Episodic; Medicare documentation.</li>
<li>Ensures that annual competency evaluations and performance reviews are completed in the appropriate time frame.</li>
<li>Rounds on unit multiple times per day to assure needs are being met.</li>
<li>Pro-actively addresses survey and/or standards of care issues (weight loss, gradual dose reduction, pressure ulcers change of condition etc.)</li>
<li>Follow up on resident/family/employee concerns in a timely manner.</li>
<li>Plans and guides the professional development of nursing staff.</li>
<li>Daily chart audits to review all assessments; nursing summaries, MARS, treatment, flow sheets and nurse’s notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident’s response to the care and that such care is provided in accordance with the resident’s wishes.</li>
<li>Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.</li>
<li>Report any medication errors and any significant changes of resident status to the Director of Nursing/Administrator.</li>
<li>Ensure that residents who are unable to call for help are checked frequently.</li>
<li>Ensure that all personnel involved in providing care to the resident are aware of the resident’s care plan. Ensure that nursing personnel refer to the resident’s care plan prior to administering daily care to the resident.</li>
<li>Ensure that all nursing care is provided in privacy and that personnel knock before entering a resident’s room.</li>
<li>Ensure that nursing service work areas (nurse’s station, med rooms, etc.) are maintained in a clean and sanitary manner.</li>
<li>Ensure that all resident care rooms, treatment areas, etc., are maintained in a clean, safe and sanitary environment.</li>
<li>Ensure that all nursing personnel follow established policies and procedures, including appropriate dress codes.</li>
<li>Assist the DON in investigations of all allegations of resident abuse.</li>
<li>Represents the DON at meetings in the community and assumes responsibility for nursing operations in the Director of Nursing’s absence.</li>
<li>Works with the staffing coordinator to ensure that shifts have adequate nursing staff to meet facility/census requirements.</li>
<li>Assists DON in preparing reports and forms in a timely manner.</li>
<li>Participates in counseling, disciplinary action and termination of staff, to the extent permitted by state practice act.</li>
<li>Answer resident call lights and ensure that all residents are checked frequently.</li>
<li>Any suspected abuse must be reported to the Director of Nursing/Administrator immediately.</li>
<li>Perform other tasks as assigned.</li>

Educational Requirements and Experience:

<li>Graduate of accredited RN School of Nursing</li>
<li>Valid RN license in State of Alabama</li>
<li>Valid CPR certification</li>
<li>One- two years of nursing experience preferred.</li>
