<div><p>We are seeking a dedicated Community Care Provider to join our team. Reporting to the Clinical Manager, you will be responsible for providing strength-based, solution-focused therapeutic services in the community. Responsibilities</b></h2><ul><li>
Provide strength-based, solution-focused supportive therapeutic services in the community.</li><li>
Deliver client-centered, individual Psychosocial Skills Rehabilitation (PSR) services in the community.</li><li>
Assess each recipient for Strengths, Resources, and Goals within first 30 days of treatment; provide Strength-based planning and treatment on an ongoing basis.</li><li>
Participate in collaborative meetings with Rehabilitation Services staff, other agencies or community and natural supports as needed.</li><li>
Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions.</li><li>
Communicate clients status on a regular basis, including changes in mood or functioning, medical issues and all crisis and health and safety issues.</li><li>
Produce effective, clinical documentation of every contact with recipient.</li><li>
Assist with the completion of 90 Day client reviews</li><li>
Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager</li><li>
Return phone calls promptly and follow all safety rules.</li><li>
Complete all yearly trainings</li></ul><h2 class="jobSectionHeader"><b>
Qualifications</b></h2><ul><li>
Masters Degree from an accredited university or college in the field of Counseling, Social Work, Psychology or Sociology.</li><li>
Ability to use computer interface to complete documentation for billing or other purposes as needed.</li><li>
Excellent communication and documentation skills.</li><li>
Ability to work collaboratively with a diverse team.</li></ul>
<p>
Offered compensation: $20/hr - $23/hr</p>
