Responsibilities:
<ul><li>Receives/responds to incoming calls from referral sources/potential clients and exchanges information to identify the clients' needs and consults with Director of Clinical Management to determine the Company's ability to meet them.</li><li>
Completes all viable referrals by setting up and carrying out pre-screening assessments, completion of pre-screening reports, and preparation of case proposals and other related pre-admission paperwork (i.e., obtains authorization for payment, coordinates availability of an appropriate treatment team).</li><li>
Oversees, directs and supervises field staff assigned in assisting with pre-screening process.</li><li>
Accesses national/state/company account information, including the account names and terms of contracts or other past payer agreements, as appropriate.</li><li>
Consults with third party representatives regarding client benefit coverage, client financial responsibility, company service authorization and specific reimbursement procedures. Presents company&rsquo;s services, interprets potential reimbursement options and negotiates reimbursement levels with third party payer.</li><li>
Contacts referral sources to advise them of case acceptance and provides information on the clinical team responsible for client's case.</li><li>
Develops/maintains a working knowledge of all services/resources provided by the Company and services available within the community. Assists in identifying alternative community service sources when company solutions are not appropriate or available.</li><li>
Maintains relationships with standard referral sources and payer case managers. Contacts identified referral sources and seeks referrals as appropriate. Records outcome of calls and keeps the SAR informed.
</li><li>Monitors/tracks referral sources' satisfaction levels, tracks/reports on conversion ratios and provides summary reports to management at requested intervals.</li><li>
Implements/maintains, with the up line management, operational processes to ensure compliance with Company policies, requirements and regulatory mandates.</li><li>
Adheres to and participates in Company&rsquo;s mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs.</li><li>
Participates in quarterly growth planning meetings/activities including discussions around staffing and recruitment needs.</li><li>
Participates in special projects and performs other duties as assigned.</li></ul>
Qualifications:
<ul><li>Bachelor's degree in a human services field or nursing field.</li><li>
Valid driver's license.</li><li>
Must have two years of experience with case management or related discipline</li><li>
Must be able to communicate both verbally and in writing.</li></ul>

Responsibilities:
<ul><li>Performs Preventative Maintenance per Preventive Maintenance Program</li><li>
Assists with vehicle home or facility inspections as directed</li><li>
Assists in pest control as required</li><li>
As assigned by the Maintenance Supervisor, completes minor repairs such as patching holes, painting, repairs, furniture,doorknobs, locks, and clearing obstructions in drains and toilets. Repairs washers, dryers, dishwashers, small appliances</li><li>
Replaces light bulbs and cover plates</li><li>
Assists with any vehicle repair</li><li>
Required to make repairs as needed w/out instruction</li><li>
May complete purchase order ensuring receipt is obtained</li><li>
Clears driveways and sidewalks of snow and uses appropriate materials to safeguard against slipping on ice</li><li>

Qualifications:

Maintenance Technician

Responsibilities:
<ul><li>Direct, manage and administer the coordination and service delivery components of programs that service individuals/consumers in a group home setting</li><li>
Supervise the delivery of service and ensure individuals and consumers are receiving the highest quality care.</li><li>
Routinely observe service delivery on-site and monitor for demonstration of knowledge of individual/consumer health needs, behavior management techniques, and emergency procedures</li><li>
Serve as a liaison between the community and agencies in the service delivery system, family/guardians and the agency</li><li>
Ensure all homes under supervision are environmentally maintained, in strict compliance with all state/federal guidelines and licensure requirements and company policy at all times</li><li>
Coordinate investigations of serious incidents and alleged abuse allegations, including appropriate reports to required agencies</li><li>
Serve as on-call support for group homes</li><li>
Ensure payroll and billing is completed accurately and timely</li><li>
Oversee the Accounts Payable for group homes</li><li>
Monitor and assure compliance with monthly, quarterly, and yearly financial goals to ensure services are provided as indicated in each person&rsquo;s Individual Program Plan</li><li>
Coordinate Incident/Accident Review process, monitor for patterns, and provide feedback for action necessary to prevent incidents in future</li><li>
Serve as member of agency management team</li><li>
Hire, train, evaluate, and monitorany other assigned personnel complete timely Performance Reviews to ensure employees are productive, accountable, and successful in their positions</li><li>
Conduct and/or monitor training for all staff in least restrictive techniques, behavior management, active treatment, client rights, prevention of abuse/neglect, documentation/data collection, emergency procedures, and other areas as needed</li><li>
Other duties as assigned</li></ul>
Qualifications:
<ul><li>Degree in Human Services field or related field and minimum of two years working directly with Elderly and Developmentally Disabled required, or equivalent combination of education and experience</li><li>
Two years supervisory experience preferred</li></ul>

Qualifications<br>
<ul><li>Bachelor&rsquo;s degree in human services, social work, psychology, or a related field required.</li><li>A minimum of two (2) years of professional experience supporting individuals with intellectual and/or developmental disabilities (IDD), or in a related human services discipline.</li><li>Preferred: Experience in a supervisory, leadership, or program management role within the human services or disability services field.</li><li>Valid driver&rsquo;s license</li><li>Ability to communicate verbally and in writing.</li><li>Must have good organizational skills and attention to detail, proven supervisory skills, and be capable of working flexible hours</li><li>Must be able to travel between service sites, staying overnight as necessary.</li></ul></div><div>
