Overview Join our team as a Targeted Case Manager (TCM), where you will play a vital role in assisting the targeted population in gaining needed medical, educational, social, and other support services. This important role offers the opportunity to make a meaningful difference by helping clients through healthcare journeys, ensuring seamless transitions, and optimizing outcomes. Your expertise in behavioral health will be instrumental in delivering exceptional client care and support.Duties<ul><li>Complete a written comprehensive ongoing assessment, which has face-to-face contact with the child, adult, their family, or other collateral needed to determine the individual&rsquo;s needs;</li><li>Participation in the development of the individual&rsquo;s case plan</li><li>Coordination of and arranging for needed services as identified in the individual&rsquo;s case plan</li><li>Assisting the individual and their family or person in custodial control in accessing needed services (both Medicaid or non-medical) as provided by a multiplicity of agencies and programs;</li><li>Monitoring the individual&rsquo;s progress through the full array of services.</li><li>Performing advocacy activities on behalf of the individual. The case manager may intercede to ensure appropriate, timely, and productive treatment modalities</li><li>Providing crisis assistance (i.e. intervention on behalf of the individual, making arrangements for emergency referrals and treatment, and coordinating any other needed emergency services).</li><li>Removing barriers that prohibit access to the recommended programs or services;</li><li>Performing follow-up on services rendered to ensure the services are received and meet the individual&rsquo;s needs.</li></ul>Experience: A qualified case manager meets the following minimum qualifications:<ul><li>A Bachelor of Arts or Bachelor of Science degree in any of the social/behavioral sciences or related fields from an accredited institution; and</li><li>One (1) year of post-bachelor experience working directly with the targeted population or performing case management services.</li><li>Completion of all Kentucky-approved TCM trainings</li></ul>Pay: $2,000.00 - $6,700.00 per monthBenefits:<ul><li>Flexible schedule</li></ul>Work Location: Hybrid remote in Somerset, KY 42503
