Job Summary We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our dedicated healthcare team. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working with a multidisciplinary team committed to excellence.Duties<ul><li>Conduct thorough physical examinations and health assessments for diverse patient populations, including pediatrics, geriatrics, and individuals with complex medical conditions.</li><li>Develop and implement individualized care plans, incorporating evidence-based practices and patient preferences.</li><li>Manage acute and chronic illnesses through medication management, diagnostic evaluation, and health coaching.</li><li>Utilize electronic health record (EHR) systems such as Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently.</li><li>Perform procedures including suturing, IV infusion, catheterization, phlebotomy, and sterile processing techniques as needed.</li><li>Coordinate with specialists for referrals or specialized procedures like gastroscopy or spinal tap when necessary.</li><li>Provide triage services via telehealth or in-person visits to assess urgent needs and prioritize care delivery effectively.</li><li>Educate patients on disease prevention, medication administration, nutrition, and lifestyle modifications to promote wellness.</li><li>Assist with discharge planning, case management, and utilization review to ensure comprehensive patient support across settings such as hospitals, nursing homes, assisted living facilities, and home care.</li><li>Maintain compliance with HIPAA regulations and ensure accurate medical documentation in accordance with NCQA standards and CPT coding guidelines.</li></ul>Experience<ul><li>Valid Family Nurse Practitioner license with current certification from the American Association of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC).</li><li>Demonstrated experience working in primary care settings with diverse patient populations including pediatrics, geriatrics, behavioral health, or acute care environments.</li><li>Proficiency with EMR/EHR systems such as Epic, Athenahealth, Cerner, or eClinicalWorks is essential.</li><li>Prior experience performing procedures like suturing, dermal fillers (nice-to-have), IV insertion/infusion therapy, catheterization, and basic life support certification required.</li><li>Experience managing complex cases involving hospital medicine, ICU or trauma center environments (Level I/II Trauma Center experience preferred).</li><li>Knowledge of ICD-10/ICD-9 coding systems along with CPT coding for billing purposes is highly desirable.</li><li>Familiarity with telehealth platforms to deliver remote patient care effectively.</li><li>Background in specialties such as gastroenterology, geriatrics, palliative medicine including hospice care or occupational health is advantageous.</li><li>Strong understanding of physiology knowledge and clinical research methodologies enhances the quality of patient assessments and treatment strategies.
