As a Social Service Associate, you will play a key role in providing exceptional long-term care, sub-acute care, and memory care. RESPONSIBILITIES
Including, but not limited to:
<ul><li>Conduct discharge planning, behavior, and care plan meetings.
</li><li>Assist residents and families with coping with the new environment and changes to physical or mental capacity, end-of-life concerns, and the grieving process.
</li><li>Work directly with the patient, family and other team members to outline the goals at admission and create a plan to meet those goals.
</li><li>Responsible for assessing our resident's psychological status during admissions and other intervals during residence.
</li><li>Assessment of our residents' psychological status during Admission.
</li><li>Knowledgeable in PASSRS, MDS, Annuals, and Quarterlies during their residency.
</li><li>Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.
</li><li>All other duties as assigned.
</li></ul>SKILLS
<ul><li>BSW or related field is required.
</li><li>Prior work with a geriatric population is also a plus.
</li><li>Must be a licensed Social Worker in the Commonwealth of MA.
</li><li>Two years of Social Work experience in long-term care or healthcare facility preferred but not required.
</li><li>Previous management experience is also a plus.
</li><li>Candidates should show evidence of creativity, organizational skills and cheerful personality.
</li><li>Previous experience in long-term care or other healthcare settings is a plus.
</li><li>Experience working with Alzheimer's/Dementia patients a plus.
</li><li>Must be capable of maintaining regular attendance.
</li><li>Good verbal and written communication skills.
</li><li>Must be able to maintain confidentiality regarding employee, resident and company proprietary information.
</li><li>Must have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levels.
</li><li>Must possess computer skills of the types and at a level necessary to accomplish the job.
</li><li>Knowledge of HIPPA, and state and federal regulations.
</li><li>Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
</li><li>Strong organizational skills to manage multiple tasks efficiently.
