<div>Job Summary:
The MDS Coordinator prepares individualized care plans for Health Care Center residents and patients according to State and Federal regulations; ensures that these care plans are followed and updated as needed. Monitors quality of care standards according to the CMS 5-star quality of care indicators.
Essential Job Functions:<ul><li>
Sets the MDS schedule and assures Minimum Data Set forms are completed in a timely manner by overseeing multi-disciplinary team.
</li><li>Ensures resident care is delivered as ordered according to Federal and State guidelines by making rounds and reviewing residents&rsquo; charts periodically.</li><li>
Ensures individual care plan approaches are being carried out or changed as needed by interacting and communicating daily with the patient staff.</li><li>
Ensures weekly care plan meetings are performed on time by the multi-disciplinary team.</li><li>
Supports proactive change by researching health, safety, and continuing education information related to nursing. Seeks continuous improvement of Salemtowne&rsquo;s nursing care.</li><li>
Supports and promotes Salemtowne Shines culture and philosophy.7. Performs other duties as assigned.</li><li>
Maintains safe, secure, and healthy work environment by following safety standards and procedures; and complies with legal regulations.</li><li>
Support the organization's mission and values by becoming familiar and acting in accordance with the policies and procedures as outlined in Salemtowne's Employee Handbook.</li><li>

Required Skills/Abilities:
<ul><li>In-depth knowledgeable of medical coding: ICD-10
</li><li>Knowledge of OSHA compliance standards.</li><li>
Strong verbal and written communication skills.</li><li>
Ability to model person-centered practices and provide direction to other team members as needed.
</li><li>Knowledgeable of Medicare and Medicaid regulations.</li><li>
Proficiency in Microsoft Office 365 (Outlook, Word, Excel, Power Point, etc.).</li><li>
Sound analytical and research skills.</li><li>
Ability to work independently and self-manage.</li><li>
Excellent time management and execution.</li></ul>
Education and Experience:<ul><li>
Graduate of an accredited Licensed Practical Nurse (LPN) or Registered Nurse (RN) educational program.
</li><li>Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in good standing with NC Board of Nursing.</li><li>
Two (2) years of Minimum Data Set and PDPM experience.</li></ul>
Physical Requirements:
<ul><li>Must be able to lift, carry, pull, and/or push up to 50 lbs.
</li><li>Normal sitting, pulling/pushing, kneeling, stooping, bending, reaching, talking, and hearing.
</li><li>Standing and sitting for long periods of time.
</li></ul>Work Environment:
Work in a clean, well-lighted facility. Possible work hazards include bloodborne pathogens, airborne particles, moving parts, muscular pains, and strong offensive odors.
