</ul>Position Overview: As a Palliative/Hospice Nurse Practitioner at Salus Hospice, you will work closely with a physician and interdisciplinary team to provide compassionate, patient-centered care. You'll help ensure hospice patients meet the face-to-face visit requirements and will support the team in delivering exceptional care to patients and their families.

Key Responsibilities:
<ul>
<li>Conduct face-to-face patient visits to evaluate continued hospice eligibility in line with new regulations.
</li>
<li>Collaborate with physicians and the interdisciplinary team to share clinical findings and adjust care plans.
</li>
<li>Perform bedside evaluations, creating comprehensive care plans tailored to the patient&rsquo;s needs.
</li>
<li>Provide direct hospice care, supporting both the patient and family through sensitive and crucial moments.
</li>
<li>Document all findings per CMS guidelines, ensuring accurate compliance.
</li>
<li>Consult with supervising physicians and collaborate with healthcare providers within and outside of Salus Hospice.
</li>
</ul>What We&rsquo;re Looking For:
<ul>
<li>2+ years of recent experience in hospice, or palliative care
</li>
<li>Expertise in providing care to patients with end-stage disease.
</li>
<li>Master&rsquo;s degree in Nursing preferred.
</li>
<li>Active Nurse Practitioner certification in California.
</li>
<li>California state licensure to practice as a Nurse Practitioner.
</li>
<li>Willingness to travel within a large geographic area to visit patients.
</li>
</ul>Qualifications:
<ul>
<li>Experience in delivering high-quality, compassionate hospice care.
</li>
<li>Deep understanding of end-of-life care and regulations.
</li>
<li>Ability to follow Standardized Procedures based on guidelines established by the Board of Registered Nursing Practice Act.</li> 
</ul>

We are looking for CHHAs who have a compassionate heart for those in Hospice. Our patients come first and our team is our most valuable asset! 
 
This is a PER DIEM opportunity, could work into full time!</div>

CHHA Requirements:

<li>Experience working in Home Health or Hospice is helpful</li>
<li>Ability to work early morning hours starting between 6 am -7 am</li>
