Samaritan Daytop Village operates transitional housing programs for single adults and families experiencing homelessness across all five boroughs of New York City. The Case Manager works directly with clients to conduct interviews, evaluate their needs, and collaboratively develop Independent Living Plans. This includes identifying and leveraging community resources, coordinating referrals to services such as healthcare, education, and legal support, and assisting clients in relocating to permanent housing. The role also involves maintaining detailed documentation, monitoring client progress, and advocating on their behalf to help remove barriers to stability and self-sufficiency.

Qualifications:
<div><ul><li>High School Diploma or equivalent required; Associate or Bachelor&rsquo;s Degree in Human Services, Social Work, or a related field strongly preferred</li><li>
At least 1 year of experience in case management or social services working with individuals experiencing homelessness, substance use, mental health challenges, or other complex needs</li><li>
Strong interpersonal skills and the ability to build trust with individuals from diverse backgrounds</li><li>
Clear and effective communication skills for conducting interviews, documenting progress, and coordinating with external partners</li><li>
Ability to maintain accurate records and meet reporting deadlines</li><li>
Commitment to maintaining client confidentiality and adhering to professional ethics</li><li>
Familiarity with New York City&rsquo;s housing, healthcare, legal, childcare, and educational resources</li><li>
Problem-solving skills and a supportive approach to helping clients overcome challenges</li><li>
Organized and detail-oriented, with the ability to manage multiple responsibilities</li><li>

The Case Manager role is a full-time non-exempt role with an hourly rate of $25.06 (annualized at 35 hours a week to $45,619) with a full benefits package.  </b><b>The Role</b></p></div><div>
Under the general direction of the Assistant Program Director, the Senior Case Manager is responsible as a senior person for ensuring the smooth operation and coordination of all case management services following all program goals, targets, and performance outcomes. Provides case management, and advocacy services to an assigned caseload and participates as a member of an interdisciplinary treatment team to facilitate the client recovery/rehabilitation process.
</div>Responsibilities:

What You Will Do</b></p></div><ul><li>
Coordinates, guides, and supervises activities of direct reporting staff.</li><li>
Provides direct supervision of Case Managers.
</li><li>Communicates all aspects of contract requirements to staff.</li><li>
Leads and participates in administrative and staff meetings as required.</li><li>
Troubleshoots problems and makes decisions in accordance with program policies, procedures, and protocols.</li><li>
Monitors clients&rsquo; progress toward their goals and documents via progress notes.</li><li>
Conducts periodic reviews of all case documentation to ensure accuracy, completeness, and clarity.</li><li>
Conducts chart auditing of client files.</li><li>
Directly manages a case portfolio of approximately 25 clients. Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.</li><li>
Provides individual counseling and facilitation of caseload group meetings and other assigned groups.
</li><li>Provides psycho-education and counseling germane to chemical dependency and relapse prevention.</li><li>
Administers appropriate behavioral interventions as needed.</li><li>
Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition, and discharge/continuing care planning collaboratively with individual clients.</li><li>
Attends case conferences and other meetings as scheduled.</li></ul>
Qualifications:

Who You Will Be</b></div></div><ul><li>
Someone with a High School Diploma or Equivalent with Five (5) years of experience preferably supervising Case Managers within transitional/shelter programs or related setting
</li></ul><div><b>OR</b></div><ul><li>
Associates Degree in Human Services or related field with at least Two-Three (2-3) years of experience supervising Case Managers within transitional/shelter programs or related settings
</li></ul><div><b>OR</b></div><ul><li>
Bachelor&rsquo;s Degree in Human Services with at least One-Two (1-2) years of experience supervising Case Managers within transitional/shelter programs or related settings.</li><li>
Working knowledge of substance abuse treatment modalities and client self-help/support modalities.</li><li>
Available to work a flexible schedule as needed in response to participant and staff needs.
