JOB SUMMARY:: 
A Sapphire Receptionist will perform the tasks needed to manage the front office in an efficient manner. This will include greeting people, answering phones, handling the intercom, giving appropriate information, and assisting the Executive Director/Administrator.
 
RESPONSBILITIES: 
<ul>
<li>Answer phone calls and represent the facility in a professional manner</li>
<li>Responding to resident questions or inquiries</li>
<li>Responding to emergency calls from residents</li>
<li>Typing communications</li>
<li>Creating new files as needed</li>
<li>Keeping information updated and communicated</li>
<li>Assisting facility staff with special projects and or assignments</li>
<li>Assist in the dining room when needed</li>
<li>Conducts facility tours for potential residents and /or their families</li>
<li>Assists with organizing special events, prepares sign-in sheets, and assists with coordinating public relations</li>
<li>Greets new residents and coordinates upcoming appointments</li>
<li>Be acquainted with emergency and disaster plans and equipment</li>
<li>Be available and visible; interact with residents</li>
<li>Record and report significant information on a potential resident's visit or call</li>
<li>Assist in the collection of monthly rent from residents</li>
<li>File bills and send out bills when asked</li>
<li>Coordinate transportation of residents as needed</li>
<li>Other duties as assigned</li>

JOB SUMMARY: 
A Registered Nurse (RN) in our Skilled Nursing Facility is responsible for delivering high-quality, person-centered care to residents, coordinating with interdisciplinary teams, and ensuring compliance with all regulatory standards. This role involves assessing resident needs, implementing individualized care plans, administering medications and treatments, and providing leadership to nursing staff. The RN plays a crucial role in enhancing the health and quality of life for residents.
 
RESPONSIBILITIES: 
<ul>
<li>Conduct comprehensive assessments of residents upon admission, change in condition, and periodically as required.</li>
<li>Participate in care plan meetings with residents, families, and the care team to discuss goals, progress, and necessary adjustments.</li>
<li>Provide direct nursing care to residents, including administering medications, treatments, wound care, and other necessary interventions.</li>
<li>Monitor and document residents' health status, including vital signs, symptoms, and responses to treatment.</li>
<li>Assist with activities of daily living (ADLs) as needed, such as bathing, dressing, grooming, and feeding.</li>
<li>Ensure all care provided is consistent with the resident's care plan and nursing standards of practice.</li>
<li>Prepare, administer, and document prescribed medications and treatments accurately and safely.</li>
<li>Monitor residents for adverse reactions and medication effectiveness, and report concerns.</li>
<li>Maintain accurate and up-to-date medication administration records (MARs) and ensure proper storage and handling of medications.</li>
<li>Serve as a liaison between residents, families, physicians, and other healthcare professionals to ensure effective communication and continuity of care.</li>
<li>Report significant changes in resident conditions promptly and take appropriate action.</li>
<li>Participate in shift change reports, staff meetings, and interdisciplinary team meetings to ensure cohesive care delivery.</li>
<li>Supervise and provide guidance to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Certified Medication Aids (CMAs) and other support staff.</li>
<li>Assist in the orientation and training of new nursing staff to ensure compliance with facility policies and procedures.</li>
<li>Delegate tasks appropriately and oversee the performance of assigned duties by nursing support staff.</li>
<li>Maintain accurate, timely, and thorough documentation in residents' electronic health records (EHR) or paper charts as required.</li>
<li>Ensure compliance with federal, state, and local regulations, as well as facility policies and procedures.</li>
<li>Assist in preparing the facility for regulatory inspections and audits by maintaining high standards of care and documentation.</li>
<li>Participate in Quality Assurance and Performance Improvement (QAPI) programs to identify areas for improvement in resident care and safety.</li>
<li>Follow infection control protocols, including proper hand hygiene, use of personal protective equipment (PPE), and disposal of biohazardous materials.</li>
<li>Report and document any incidents, accidents, or safety concerns promptly and accurately.</li>
<li>Advocate for the rights, preferences, and needs of residents, promoting a respectful and compassionate care environment.</li>
<li>Other duties as assigned.</li>
</ul>
REQUIREMENTS: 
<ul>
<li>Registered Nurse (RN) Licensure in the State of practice</li>
<li>Previous experience in a skilled nursing facility, long-term care, or acute care setting preferred.</li>
<li>Strong clinical skills and knowledge of geriatric care.</li>
<li>Excellent communication, interpersonal, and organizational skills.</li>
<li>Ability to work effectively as part of an interdisciplinary team.</li>
<li>Proficiency in electronic health records (EHR) and basic computer skills.</li>
<li>Critical thinking and problem-solving abilities.</li>
<li>Ability to perform physically demanding tasks, such as lifting, bending, standing, and assisting residents with mobility.</li>
<li>Must be able to stand and walk for extended periods.</li>
