JOB SUMMARY: 
Working as a Caregiver in our Community Based Care Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with residents, their families, and care team to provide personalized care that meets each individual's unique needs and preferences.
 
RESPONSIBILITIES: 
<ul>
<li>Provide assistance with activities of daily living (ADLs).</li>
<li>Monitor and report any changes in residents physical or mental health.</li>
<li>Assists with serving resident meals.</li>
<li>Provide companionship, emotional support, and meaningful engagement to residents.</li>
<li>Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.</li>
<li>A Caregiver will maintain resident confidentiality.</li>
<li>Encourage clients to participate in activities that promote physical, mental, and emotional well-being.</li>
<li>Support clients in maintaining independence in their daily routines and decision-making.</li>
<li>Ensure a safe environment by identifying and addressing potential hazards in community setting.</li>
<li>Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.</li>
<li>Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.</li>
<li>Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.</li>
<li>Present for staff meetings and training.</li>
<li>Other duties as assigned.</li>
</ul>
REQUIREMENTS: 
<ul>
<li>Must be 18 years of age</li>
<li>NAC/CNA or NAR or HCA acquired within 120 days of hire</li>
<li>High school diploma or equivalent required.</li>
<li>Previous experience in caregiving, home care, or community-based care is preferred.</li>
<li>Ability to work independently and as part of a team.</li>
<li>Patience, empathy, and a genuine desire to help others.</li>
<li>Good organizational and time management skills.</li>
<li>Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.</li>
<li>Must be able to stand, walk, and reach for extended periods.</li>
<li>Ability to pass a background check.</li>

JOB SUMMARY:: 
Working out of our Community Based Care facility, the Med Tech is responsible for administering medications to our residents in accordance with the established policies and procedures as directed by the supervisor. Sapphire will provide the training and resources to keep our employees engaged and maintain top quality care.
 
RESPONSIBILITIES:

<li>Provide residents with routine daily nursing care and services</li>
<li>Administer medications as ordered and in accordance with state and federal regulations and facility policies and procedures</li>
<li>Seek licensed staff assistance as needed</li>
<li>As a Med Tech you will follow the plan of care and chart all appropriate documentation</li>
<li>Maintain patient confidentiality</li>
<li>Communicate both orally and written with staff, residents, family members, etc.</li>

JOB SUMMARY: 
A Medical Technician (Med Tech) will play an essential role in delivering compassionate care to residents within a community-based care setting. This position involves providing direct care, administering medications, monitoring patient health, and supporting overall wellness. The ideal candidate will have a strong sense of responsibility, a compassionate approach, and a commitment to high-quality care.
 
RESPONSIBILITIES: 
<ul>
<li>Maintain patient confidentiality</li>
<li>Fosters a supportive, compassionate environment for our residents and their families</li>
<li>Medication Administration: Prepare and administer medications to residents as prescribed, following all state and federal regulations, facility policies and procedures and maintaining accurate medication records.</li>
<li>Report changes in residents' conditions to appropriate team members.</li>
<li>Assistance with Daily Living: Support residents with daily activities such as bathing, dressing, and grooming, ensuring dignity and comfort.</li>
<li>Care Coordination: Collaborate with nursing and other healthcare professionals to create and maintain individual care plans for each resident.</li>
<li>Documentation: Accurately document all care, medications, and any observed changes in residents’ conditions in accordance with company policies.</li>
<li>Emergency Response: Act promptly and calmly in emergencies, following established protocols to ensure resident safety.</li>
<li>Family Communication: Communicate with residents' families regarding health status, medication changes, and overall care updates, fostering trust and transparency.</li>
<li>Compliance: Maintain compliance with state and federal regulations, as well as company policies regarding healthcare and safety standards.</li>

REQUIREMENTS:

<li>Previous experience in healthcare setting, preferably in assisted living, long-term care or community based care.</li>
<li>Ability to work independently and as part of a team.</li>
