Responsibilities:

SUMMARY:</b> The Registered Nurse provides high quality individualized direct patient care utilizing the skills and knowledge of the profession. Please note this is a <b>RN phone triage</b> position located in our administrative office. Contact with patients will be via phone, not face to face.</div><div></div><div><b><br>
Job Duties &amp; Essential Functions:
</b></div><ul><li>Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider.</li><li>
Triage patient phone calls and troubleshoot medication management issues.</li><li>
Demonstrates competency in answering phones, assembling charts, patient registration, making appointments, computer data entry and retrieval of reports.</li><li>
Must safely perform all patient care responsibilities in accordance with professional standards and the organization&rsquo;s policies and procedures.</li><li>
Develop and organize a plan of care in consultation with treating physician, as indicated within the various subspecialties of the Department.</li><li>
Must effectively determine and set priorities to meet patient needs.</li><li>
Provide proper and accurate documentation of patient care in the patient medical record.</li><li>
Coordinate any services or accommodations necessary (e.g. sign language interpreter) to ensure effective communications between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers.
</li><li>Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals.</li><li>
Administer medications, treatments, and contrast agents according to the specific orders of the ordering physician, and document in the patient record.</li><li>
Maintains infection control precautions and identifies potential infection control problems.</li><li>
Provides in-service education to members of the interdisciplinary team within the Department.</li><li>
Participate in Quality Assurance activities within the Department.
</li><li>Participates in annual recertification training, compliance training, and HIPAA training, as well as all other mandatory in-service programs.</li><li>
Conforms to policies and regulations governing patient rights and confidentiality of information.</li><li>
Perform all other related duties as assigned by management.</li></ul>
Qualifications:

Required Education &amp; Qualifications:</b></div><ul><li>
The candidate must have an Associate&rsquo;s degree in Nursing. (New York State BSN in 10 Law - Must attain Bachelor&rsquo;s in Nursing or higher within 10 years of initial licensure. Current RNs, as well as those currently enrolled or pending acceptance into a program preparing registered nurses effective date of this act (which is Dec. 19, 2017) are grandfathered in.)</li><li>
Basic Cardiac Life Support (BCLS) required.
</li><li>Current registration to practice as a Licensed Registered Nurse in New York State.
</li><li>Must have excellent communication skills.</li></ul><div><b>

Bachelor&rsquo;s degree in Nursing.</li><li>
Advanced Cardiovascular Life Support (ACLS) preferred.</li></ul><div><b>

SUMMARY:</b> The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of the Practice. This position also performs clerical duties, acts as receptionist, maintains medical records and communicates with all departments.</div><div></div><div><b><br>

Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider.</li><li>
Triage patient phone calls and troubleshoot medication management issues</li><li>
Documents nursing observations, interventions, and patient&rsquo;s response to treatment in patient records.</li><li>
Demonstrated competency in performing venipuncture/administering shots for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected.</li><li>
Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN).</li><li>
Accurately performs and records vital signs, weight and height, point of service blood glucose testing, and promptly reports abnormalities to the medical provider or Registered Nurse (RN) on duty.</li><li>
Utilizes appropriate techniques when collecting routine urine specimens and specimens for C/S.</li><li>
Appropriately instructs patients on clean catch specimens.</li><li>
Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure.</li><li>
Verbalizes knowledge of and adherence to OSHA standards for blood borne pathogens when handling blood and body fluids.</li><li>
Maintains standard precautions in the provision of patient care.</li><li>
Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis.</li><li>
Prepares and processes requisitions for routine supplies in accordance with agency procedures.</li><li>
Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager.</li><li>
Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports.</li><li>
Copies, prepares records, and stores and maintains appropriate records.</li><li>
Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions.</li><li>
Conforms to policies and regulations governing patient rights and confidentiality of information.</li><li>
Adheres to protocol for patient flow (1. Reception, 2. Registration, 3. Labs, 4. Escort to Provider).</li><li>
Perform all other related duties as assigned by management.</li></ul>
Qualifications:

Graduate of an accredited school of Practical Nursing.</li><li>
Currently registered in New York State as a Licensed Practical Nurse (LPN).

</li><li>Demonstrated competency in performing venipuncture for collection of blood specimens.</li></ul><div><b>
Preferred Qualifications:
</b></div><ul><li>Three years&rsquo; experience in ambulatory care setting required as a LPN</li></ul><div><b>
