Shift: Full-Time | All three shifts (6:00 AM &ndash; 2:00 PM, 2:00 PM &ndash; 10:00 PM, 10:00 PM &ndash; 6:00 AM) 
Salary: $20/hour

About the Role: 
Seagate Rehabilitation and Nursing Center is seeking a compassionate and dedicated Home Health Aide to escort residents to and from clinic and dialysis appointments while ensuring their safety, comfort, and well-being. This role requires a proactive and professional individual who can represent our facility with integrity both on and off-site.

Key Responsibilities:
<ul>
<li>Escort residents to and from clinic and dialysis appointments.
</li>
<li>Arrive at work 1 hour prior to scheduled clinic appointments.
</li>
<li>Be in full nursing uniform with ID badge visible at all times.
</li>
<li>Assist residents with preparing for appointments.
</li>
<li>Provide residents with their out-on-pass snacks.
</li>
<li>Monitor residents to prevent falls or altercations; intervene immediately to prevent incidents.
</li>
<li>Report all incidents or accidents to the Clinical Coordinator or ADNS immediately.
</li>
<li>Obtain appropriate reports and paperwork for the resident&rsquo;s clinic visit.
</li>
<li>Verify each resident&rsquo;s DNR/DNI status and ensure non-hospital DNR forms are available when required.
</li>
<li>Act as a proactive advocate for residents at clinic visits, communicating any issues to the facility promptly.
</li>
<li>Represent the facility professionally, adhering to all policies, procedures, and abuse laws.
</li>
<li>May be required to attend more than one clinic visit per day depending on facility needs.
</li>
<li>Coordinate with the clinic coordinator regarding unit assignments before or after clinic visits.
</li>
<li>Report directly to the Clinic Coordinator or ADNS.
</li>
</ul>Requirements:
<ul>
<li>High school diploma or equivalent preferred.
</li>
<li>Current NYS HHA certification required.
</li>
<li>CNA certification preferred.
</li>
<li>Minimum 1 year of experience with EMR (Sigma preferred).
</li>
<li>Prior nursing home experience preferred.
</li>
