The Seneca Nation Health System provides a comprehensive range of primary and preventive healthcare services for both acute and chronic conditions. <div>The incumbent ensures that safe transportation is provided to Seneca Nation Health System (SNHS) patients. Reports directly to Transportation Coordinator. 
GENERAL RESPONSIBILITIES: 

<div>
<ul><li>Inspects vehicle for defects and safe operating conditions daily and provides written documentation of each inspection.</li></ul>
<ul><li>Drives vehicle to transport patients to and from the health center or other health facilities with pick up at the patients residence.</li></ul>
<ul><li>Represents the organization with positive and professional interactions.</li></ul>
<ul><li>Adheres to all traffic laws and regulations in a conscientious and safe manner.</li></ul>
<ul><li>Regulates heating, lighting, and ventilation systems for patient comfort.</li></ul>
<ul><li>Assists patients boarding and exiting vehicle, if necessary.</li></ul>
<ul><li>Ensures vehicle is clean and well-maintained at all times.</li></ul>
<ul><li>Makes deliveries and pick-ups of health related packages and documents.</li></ul>
<ul><li>Maintains records of patient pick-up and drop-off times, trips and miles driven.</li></ul>
<ul><li>Reports any vehicle malfunctions to supervisor and fleet management.</li></ul>
<ul><li>Works to promote cooperative staff efforts to provide and maintain high quality service to patients, physicians and other staff to assure quality patient care.</li></ul>
<ul><li>Communicates effectively with patients to provide information and assistance.</li></ul>
<ul><li>Upholds and promotes the value and integrity of the Seneca Nation Health System&rsquo;s mission statement. Will promote SNHS in a positive manner.</li></ul>
<ul><li>Provides care through a trauma-informed lens by incorporating key principles of both physical and psychological environment safety, trustworthiness and transparency, collaboration of decision-making during treatment plan developments, empowerment of patients by recognizing and building upon individual strengths, as well as recognizing and respecting the diversity of cultural backgrounds and sensitivity.</li></ul>

QUALIFICATIONS: 
<ul>
<li>
Valid and clean N.Y.S. Class E (livery/taxi) driver&rsquo;s license OR CDL with Passenger endorsement, required.</li>
<li>Two (2) years of previous experience in commercial driving, such as school bus driving or paratransit, is preferred.</li>
<li>Ability to lift heavy objects, as some passengers may require assistance with mobility devices.</li>
<li>Minimum age of 21 years due to insurance requirements.</li>
<li>Must be able to work flexible hours, including early mornings and late afternoons.</li>
<li>Must have a current (within 30 days) driving abstract.</li>
<li>Must pass the SNI NTSI class after a successful 90 probation period.</li>
<li>Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.</li></ul></div>

 Basic Function
 
 
 
 Incumbent reviews, analyzes, and codes diagnostic and procedural information that determines Medicare, Medicaid, and private insurance payments. Ensures compliance with established coding guidelines, third party reimbursement policies, regulations and accreditation guidelines.
 
 
 
 General Responsibilities
 
 
 
 <ul>
 <li>
 Abstracts all necessary information and sequences and assigns codes (ICD-10, CPT, and HCPCS), which most accurately describe each documented diagnosis, surgical procedure and special therapy/procedure according to established guidelines, and to identify secondary complications and co-morbid conditions.
 </li>
 <li>
 Determines the final diagnoses and procedures as stated by the provider are valid and complete. Ensures documentation is complete to justify treatment and diagnoses.
 </li>
 <li>
 Quantitative analysis &ndash; Performs a comprehensive review for the record to ensure the presence of all component parts such as: patient and record identification, signatures and dates where required.
 </li>
 <li>
 Quantitative analysis &ndash; Evaluates the record for documentation consistency and adequacy. Ensures that final diagnosis accurately reflects the care and treatment rendered. Reviews the records for compliance with established third party reimbursement agencies and special screening criteria.
 </li>
 <li>
 Analyzes documentation to ensure the appropriate evaluation and management (ER&amp;M) levels are assigned using the correct CPT code.
 </li>
 <li>
 Processes and accurately data enters assigned codes into the electronic health record and billing system; generates reports as needed to ensure completion. Follows up routinely on outstanding encounters that need to be coded.
 </li>
 <li>
 Conducts chart reviews and audits for documentation reviews and provides feedback and education to clinical providers and other staff.
 </li>
 <li>
 Serves as a coding subject-matter expert and provides answers to questions regarding coding and documentation.
 </li>
 <li>
 Queries providers when documentation and diagnoses need clarification.
 </li>
 <li>
 Attends mandatory staff meetings and in-services, including training to stay current in position and/or department.
 </li>
 <li>
 Participates in quality assurance/improvement/control activities.
 </li>
 <li>
 Follows all policies and procedures of the department, Seneca Nation, and Seneca Nation Health System.

 Knowledge, Skills, &amp; Abilities

 Advanced knowledge of diagnostic and procedural coding systems, medical terminology, abbreviations, minor medical procedures, anatomy and physiology, major disease processes, pharmacology, and the metric system.

 Knowledge of official coding conventions and rules established by American Medical Association (AMA), and Center for Medicare and Medicaid Services (CMS) for assignment of diagnostic and procedural codes.

 Knowledge of electronic health records systems.

 Knowledge of Microsoft Office Suite, specifically Word, Excel, and Power Point.

 Must have data entry skills.

 Must possess and maintain current CPC, COC, CCS, CCS-P, CCA, RHIA, OR RHIT certification.

BASIC FUNCTION: 

Responsible for the provision of nutrition services for the Seneca Nation Health System (SNHS). Coordinates nutrition services with other health services and with existing nutrition programs of other agencies. Plans, prepares, and conducts in-service educational programs in nutrition. 
GENERAL RESPONSIBILITIES:

<ul><li>Plans, develops and evaluates the nutrition component of the program which includes determining needs and barriers of individuals and the population. Assists in developing policies, standards and services in accordance with nutrition services.</li></ul>
<ul><li>Creates nutrition plans to improve the health and physical conditions of the patients considering the household and other contributing factors to food access. Analyzes the patient&rsquo;s health status, goals and dietary restrictions and creates a suitable nutritional plan for individual patients considering their life circumstances and chronic health conditions.</li></ul>
<ul><li>Tracks patient&rsquo;s dietary intake and progress.</li></ul>
<ul><li>Educates patients and families on how to maintain a balanced and healthy diet.</li></ul>
<ul><li>Connects patients with other needed health services such as fitness training and behavioral health counseling.</li></ul>
<ul><li>Promotes nutrition through community wide presentations and awareness programs.</li></ul>
<ul><li>Stays up to date on the latest nutrition research.</li></ul>
<ul><li>Provides nutrition consultation to professional staff and other Seneca Nation programs.</li></ul>
<ul><li>Cooperates with and initiates plans with appropriate community agencies for provision of food assistance for families.</li></ul>
<ul><li>Prepares reports and summaries as required.</li></ul>
<ul><li>Performs duties with sensitivity to community needs and cultural differences.</li></ul>

QUALIFICATIONS: 
<ul>
<li>
Master&rsquo;s degree in Nutrition, Dietetics or related field is required.</li>
<li>Registered Dietician Nutritionist Certificate, RDN, from the Commission of Dietetic Registration (CDR) required or willingness to obtain within 18 months of hire.</li>
<li>Must have or obtain Certified Diabetes Care and Education Specialist (CDCES) certification within 5 years of employment.</li>
<li>Public health experience preferred.</li>
<li>Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.</li></ul></div>
