 Basic Function
 
 
 
 The incumbent performs oral prophylaxis and periodontal therapy in the prevention of or protective treatment of oral disease. Supervises and coordinates community-based programs and provides off-site direct dental care and outreach to high-risk patient groups (children, elders, etc.).
 
 
 
 General Responsibilities
 
 
 
 <ul>
 <li>
 Performs oral prophylaxis and scaling in the prevention of oral disease.
 </li>
 <li>
 Takes and develops diagnostic radiographs.
 </li>
 <li>
 Observes the condition of structures and tissues within the oral cavity, recording findings and reporting conditions to the Dentist.
 </li>
 <li>
 Provides onsite and offsite fluoride treatments as a preventive measure to children up to sixteen years of age.
 </li>
 <li>
 Provides detailed instruction to all patients on the methods and importance of good oral hygiene, reflecting on each patient and knowledge specific to the various clinical or community program served.
 </li>
 <li>
 Provides direct off-site preventive and routine care in conjunction with coordinated community based prevention and patient education programs, including school-based preventive care delivered at ECLC Health Offices.
 </li>
 <li>
 May be required to assist Dentist in the absence of the dental assistant.
 </li>
 <li>
 Participates and completes all mandatory staff meetings, trainings and in-services.
 </li>
 <li>
 Follows all policies and procedures of the department, Seneca Nation Health System (SNHS) and the Seneca Nation.
 </li>
 <li>
 Supervises and instructs university dental interns while providing hygiene and periodontal care while on rotation with SNHS inclusive of examination and treatment planning.
 </li>
 <li>
 Oversees care management efforts of outreach project(s) including maintaining screening records and coordination with offsite healthcare staff (e.g. ECLC, SALC nursing staff, etc.), parents or guardians to ensure recommended care is received in a timely fashion.
 </li>
 <li>
 Coordinates all outreach and educational fairs on both territories, which includes preparing dental supplies for community distribution at outreach events; including ordering, stocking and maintaining inventory.
 </li>
 <li>
 Implements hygiene recall system by initiating correspondences with patients through various means of communication such as phone calls, written reminders and overdue notices to stress importance of oral health.

 Qualifications

 Completion of an accredited dental hygiene program and NYS licensing as a Registered Dental Hygienist required.

 Associates Degree in Applied Sciences required.

 <li>
 Three years&rsquo; experience at an FQHC with involvement in community-based programming is required.
 </li>
 <li>
 Five years&rsquo; experience in a non-FQHC setting with prior community-based programming is preferred.
 </li>
 <li>
 Credentialed as an adjunct clinical instructor with an affiliated Dental School (e.g. UBSDM) for supervision and instruction of University Dental Interns providing periodontal and hygiene care to SNHS patients.
 </li>
 <li>
 Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.

 Incumbent reviews, analyzes, and codes diagnostic and procedural information that determines Medicare, Medicaid, and private insurance payments. Ensures compliance with established coding guidelines, third party reimbursement policies, regulations and accreditation guidelines.
 
 
 
 General Responsibilities
 
 
 
 <ul>
 <li>
 Abstracts all necessary information and sequences and assigns codes (ICD-10, CPT, and HCPCS), which most accurately describe each documented diagnosis, surgical procedure and special therapy/procedure according to established guidelines, and to identify secondary complications and co-morbid conditions.
 </li>
 <li>
 Determines the final diagnoses and procedures as stated by the provider are valid and complete. Ensures documentation is complete to justify treatment and diagnoses.
 </li>
 <li>
 Quantitative analysis &ndash; Performs a comprehensive review for the record to ensure the presence of all component parts such as: patient and record identification, signatures and dates where required.
 </li>
 <li>
 Quantitative analysis &ndash; Evaluates the record for documentation consistency and adequacy. Ensures that final diagnosis accurately reflects the care and treatment rendered. Reviews the records for compliance with established third party reimbursement agencies and special screening criteria.
 </li>
 <li>
 Analyzes documentation to ensure the appropriate evaluation and management (ER&amp;M) levels are assigned using the correct CPT code.
 </li>
 <li>
 Processes and accurately data enters assigned codes into the electronic health record and billing system; generates reports as needed to ensure completion. Follows up routinely on outstanding encounters that need to be coded.
 </li>
 <li>
 Conducts chart reviews and audits for documentation reviews and provides feedback and education to clinical providers and other staff.
 </li>
 <li>
 Serves as a coding subject-matter expert and provides answers to questions regarding coding and documentation.
 </li>
 <li>
 Queries providers when documentation and diagnoses need clarification.
 </li>
 <li>
 Attends mandatory staff meetings and in-services, including training to stay current in position and/or department.
 </li>
 <li>
 Participates in quality assurance/improvement/control activities.
 </li>
 <li>

BASIC FUNCTION: 

Responsible for the provision of nutrition services for the Seneca Nation Health System (SNHS). Coordinates nutrition services with other health services and with existing nutrition programs of other agencies. Plans, prepares, and conducts in-service educational programs in nutrition. 
GENERAL RESPONSIBILITIES:

<ul><li>Plans, develops and evaluates the nutrition component of the program which includes determining needs and barriers of individuals and the population. Assists in developing policies, standards and services in accordance with nutrition services.</li></ul>
<ul><li>Creates nutrition plans to improve the health and physical conditions of the patients considering the household and other contributing factors to food access. Analyzes the patient&rsquo;s health status, goals and dietary restrictions and creates a suitable nutritional plan for individual patients considering their life circumstances and chronic health conditions.</li></ul>
<ul><li>Tracks patient&rsquo;s dietary intake and progress.</li></ul>
<ul><li>Educates patients and families on how to maintain a balanced and healthy diet.</li></ul>
<ul><li>Connects patients with other needed health services such as fitness training and behavioral health counseling.</li></ul>
<ul><li>Promotes nutrition through community wide presentations and awareness programs.</li></ul>
<ul><li>Stays up to date on the latest nutrition research.</li></ul>
<ul><li>Provides nutrition consultation to professional staff and other Seneca Nation programs.</li></ul>
<ul><li>Cooperates with and initiates plans with appropriate community agencies for provision of food assistance for families.</li></ul>
<ul><li>Prepares reports and summaries as required.</li></ul>
<ul><li>Performs duties with sensitivity to community needs and cultural differences.</li></ul>
<ul><li>Actively participates in case conferences and planning which may involve other health care providers.</li></ul>
<ul><li>Works to promote cooperative staff efforts to provide and maintain high quality service to patients, physicians and other staff to assure quality care.</li></ul>

QUALIFICATIONS: 
<ul>
<li>
Master&rsquo;s degree in Nutrition, Dietetics or related field is required.</li>
<li>Registered Dietician Nutritionist Certificate, RDN, from the Commission of Dietetic Registration (CDR) required or willingness to obtain within 18 months of hire.</li>
<li>Must have or obtain Certified Diabetes Care and Education Specialist (CDCES) certification within 5 years of employment.</li>
<li>Public health experience preferred.</li>
<li>Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.</li></ul></div>
