What You'll Do</b></div>

<li>Deliver personalized, high-quality care to older adults in homes and senior living settings.</li>
<li>Conduct comprehensive assessments and develop individualized care plans.</li>

<li>Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.</li>
<li>Build trusted relationships with residents, caregivers, and community partners.</li>

What You'll Bring</b></div>

<li>Active, unencumbered NP or PA license in the state you will be practicing.</li>
<li>Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).</li>

<li>Valid driver's license and reliable transportation.</li></ul><br>
