As a provider member of our team, you will leverage the virtual medicine platform to support patient self-management, while ensuring PCPs are kept aware of changes in their patient&rsquo;s health status.

The AI based technology allows for easy and efficient administration and you will spend much less time documenting and more time directing the care of your patient.</div><div>

Minimum Qualifications:</b></div><ul><li>
Board Certified Nurse Practitioner with experience in Primary Care, Family Practice and/or Geriatrics. Additional experience in Psychiatrics a strong plus.</li><li>
Minimum of Master&rsquo;s Degree in Nursing.</li><li>
APRN license in good standing in Massachusetts w/additional licensure in Florida, Maine, New Hampshire, Rhode Island, Pennsylvania, North Carolina, and Connecticut a plus.</li><li>
10+ years of experience managing complex patients.</li></ul><div>

Additional Experience Desired:</b></div><ul><li>
Systems mindset: translating assessments into coordinated tasks.</li><li>
Strong clinical reasoning + decisiveness under uncertainty.</li><li>
Comfortable in a highly dynamic environment, with a mix of regularly scheduled &ldquo;cadence&rdquo; work while responding to short notice escalations.</li><li>
Willingness to obtain licensure in all states/regions where Senscio has operations.</li><li>
Personally credentialed with Payors and Health Plans a significant plus.</li><li>
Experience/interest in telehealth service delivery.</li><li>
Passion for providing Chronic Care Management.</li></ul></div><div></div>
