MAJOR DUTIES AND RESPONSIBILITES
Description:
The Survive to Thrive program will identify and serve human trafficking survivors among immigrant populations, offering community education and outreach, provide culturally informed case management, referrals and survivor-led support.
Primary Duties and Responsibilities:<ul><li>
Provide public awareness and community outreach to educate stakeholders about human trafficking.</li><li>
Provide case management services to human trafficking survivors including information and referrals, advocacy, social emotional support and survivor led support.</li><li>
Manage a caseload of human trafficking survivors providing trauma-informed case management services.</li><li>
Engage in public awareness events and community outreach to inform key stakeholders about all forms of human trafficking so they can recognize signs, provide appropriate and compassionate support, and make referrals to Survive to Thrive and other community resources.</li><li>
Meet regularly with clients in home, community, SGA site settings, or virtually to check-in and review progress in reaching goals.</li><li>
Provide crisis support, screening and assessment.</li><li>
Determine eligibility for services, assess risk factors, strengths and needs.</li><li>
Establish short and longer-term goals.</li><li>
Through case management, the case manager will help clients understand and experience healthy, trusting relationships and build social and emotional skills to continue building healthy relationships.</li><li>
Utilize behavioral health support strategies to teach emotional regulation and coping.</li><li>
Teach new skills such as budgeting and problem-solving, provide critical information, such as the criminal justice system and rights as victims.</li><li>
Advocate for the needs of survivors and help survivors access practical supports such as education/employment, child care, legal services, health care, transportation to medical and other appointments and benefits eligibility.</li><li>
Utilize network of community referral partners to ensure client access resources.</li><li>
Manage caseload including crisis management, intake, assessment, intensive case management, emotional support, peer support and follow-up.</li><li>
Co-facilitate survivor-led mentoring and support groups.</li><li>
Engage in supervision including case debriefing, self-care and stress management.</li><li>
Participate in regular team meetings to discuss client status and anticipated needs.</li><li>
Respond promptly to client needs, coordinating with the team to ensure appropriate rendering of services and supports.</li><li>
Document all client interactions, case updates, and emergency situations accurately and in a timely manner.</li><li>
Provide ongoing support and referrals to participants as needed.</li><li>
Other duties as assigned.</li></ul>

Ability to write routine reports and correspondence.</li><li>
Ability to speak effectively before groups of clients, stakeholders or employees or the organization.</li><li>
Exercises discretion and independent judgment in executing tasks</li><li>
Must demonstrate the ability to establish helping relationships with diverse populations and handle emergency and crisis situations.</li></ul>
Requirements:

QUALIFICATIONS FOR THE JOB
Education:<ul><li>
Bachelor&rsquo;s Degree in the field of Counseling, Psychology, Criminal Justice, Social Work, Education or related program (preferred).</li></ul>
Experience:<ul><li>
Experience in case management, social services, or behavioral health.</li><li>
Strong crisis management and problem-solving skills.</li><li>
Excellent communication and organizational abilities.</li><li>
Knowledge of human trafficking, domestic violence, LGBTQIA+, community resources, and support services.</li><li>
Experience working with diverse populations and in diverse communities.</li></ul>
KEY COMPETENCIES<ul><li>
Strong collaborative, relationship building, and communication skills to effectively problem solve, achieve goals, and meet deliverables.</li><li>
Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse populations, various professionals and community groups.</li><li>

PHYSICAL REQUIREMENTS<ul><li>
Must be able to remain stationary 50% of the time.</li><li>
Must be able to frequently move inside the office to access file cabinets, office machinery, etc.</li><li>
Must be able to transport oneself to and from community visits.</li><li>
Occasionally ascends/descends stairs.</li><li>
Constantly moves to assist during program activities.</li><li>
At times it may be required to lift up to 25 lbs. for various needs.</li><li>
Frequently moves in outdoor weather conditions.</li></ul>
CONDITIONS OF EMPLOYMENT<ul><li>Ability to work varying hours Monday - Friday, Schedule may include some evenings and weekends.</li><li>Must have a valid driver&rsquo;s license and automobile insurance required for work related travel</li></ul>
STATUS: Full-time
SALARY GRADE: $50,000 annually

Case Manager

<div>Description:
<ul><li>Provides mentorship and advocacy services to youth ages 16 and 24 with a consistent caseload of 75 youth.</li><li>
Assists with recruiting and retaining 75 youth.
</li><li>Works with each youth to develop an Individualized Service Plan to address areas for improvement such as housing, academics, social emotional learning, counseling referrals, life skills, etc.</li><li>
Works with Job Developer to support employment opportunities, site visits, and retention efforts.</li><li>
Ensures files remain up-to-date and in compliance.</li><li>
Completes employability skills checklist, conduct all Illinois WorkNet modules, &amp; ensure pre/post assessment.</li><li>
Assist in-school youth to improve educational outcomes.</li><li>
Provide information and referrals to internal and external resources</li><li>
Re-engage out-of-school youth in educational activity.</li><li>
Collects, reviews, and analyzes all transcripts for youth to determine GED or High School Diploma track.</li><li>
Utilizes Labor Market Data and information to help guide youth in career and education goals.</li><li>
Works to assist youth to determine what factors put them at risk of engaging in or becoming victims of aggressive behavior and other anxieties.</li><li>
Helps to determine what methods each youth should use in order to avoid risk-taking factors and reach their goals.</li><li>
Works in collaboration within the targeted communities to cultivate relationships with local community resources.</li><li>
Reaches out and re-engage youth that have inconsistent attendance, work with families to bring resources to the home, and connect with programs.</li><li>
Conducts Home visits.</li><li>
Support outreach events, referral systems, and programs facilitated by other departments within agency.</li><li>
Transporting students in vehicle to and from program related activities may be required.</li><li>
Implements other services requested by SGA or funder.</li><li>
Maintain detailed record of activities to regularly track impact of outreach efforts and to facilitate appropriate reports to funders and government agencies.</li><li>
Enters daily attendance, case notes into ILWorknet.</li><li>
Coordinates with the Job Developer to provide ongoing program support and implementation.</li><li>
Conducts required reports and data material to SGA such as the Employability/Life Assessment, TABE Test etc.</li><li>
Ability and experience in short and long-range planning and strategy Detail oriented with the ability to multi-task and self-direct your work</li><li>

Requirements:

Must be Bilingual</li><li>
Experience working one on one with youth involved in the juvenile justice system.</li><li>
Ability to display sensitivity and responsiveness to cultural differences present in the service population</li></ul>
Experience/Education Required/Sought:<ul><li>
Bachelor's degree in social work, criminal justice or related field, preferred</li><li>
Experience working with teens and young adults, preferred</li><li>
Experience working with individuals dealing with factors that put them at-risk of becoming victims of aggressive behavior.</li><li>
