<div>General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily care and services in accordance with the resident&rsquo;s assessment and care plan, and as may be directed by your supervisors.
Essential Duties<ul><li>
Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.</li><li>
Report all changes in the residents&rsquo; condition to the Health and Wellness Director as soon as practical.</li><li>
Record all entries on notes, SPAs, and incident reports, etc., in an informative, descriptive manner.</li><li>
Report on all accidents and incidents you observe on the shift that they occur to the Executive Director and Health and Wellness Director.</li><li>
Agree not to disclose resident&rsquo;s protected health information and promptly report suspected or known violations of such disclosure to the Executive Director.</li><li>
Report any known or suspected unauthorized attempt to access community&rsquo;s information system.</li><li>
Perform only those care tasks that you have been trained to do.</li><li>
Ensure that the resident&rsquo;s room is ready for the resident (i.e., bed made).</li><li>
Greet residents and escort them to their room.</li><li>
Introduce residents to his/her roommate, if any, and other residents and personnel as appropriate.</li><li>
Make residents comfortable.</li><li>
Inventory and mark the residents&rsquo; personal possessions as instructed.</li><li>
Store resident&rsquo;s clothing.</li><li>
Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged.</li><li>
Transport residents to new rooms or to the receiving area.</li><li>
Assist with loading/unloading residents&rsquo; to/from vehicles as necessary.</li><li>
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.</li><li>
Meet with your shift&rsquo;s nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services.</li><li>
Report all complaints and grievances made by residents.</li><li>
Participate in and receive the nursing report as instructed.</li><li>
Follow established policies concerning exposure to blood/body fluids.</li><li>
Make beds as instructed.</li><li>
Put extra covers on beds as requested.</li><li>
Ensure that residents who are unable to call for help are checked frequently.</li><li>
Answer resident calls promptly.</li><li>
Check residents routinely to ensure that their personal care needs are being met.</li><li>
Assist residents with identifying food arrangements (i.e., informing residents with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).</li><li>
Keep residents&rsquo; water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident.</li><li>
Perform after meal care (i.e., remove trays, clean resident&rsquo;s hands, face, clothing, etc.).</li><li>
Receive the nursing report upon reporting for duty.</li><li>
Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.</li><li>
Follow work assignments, and/or work schedules in completing and performing your assigned tasks.</li><li>
Cooperate with inter-departmental personnel, as well as other facility personnel, to ensure that services can be adequately maintained to meet the needs of the residents.</li><li>
Notify the facility when you will be late or absent from work.</li><li>
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility&rsquo;s policies and procedures governing accidents and incidents.</li><li>
Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary).</li><li>
Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled).</li><li>
Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.).</li><li>
Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.</li><li>
Perform restorative and rehabilitative procedures as instructed.</li><li>
Assist in preparing the resident for a physical examination.</li><li>
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.</li><li>
Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).</li><li>
Report injuries of an unknown source, including skin tears.</li><li>
Observe disoriented and comatose residents. Record and report data as instructed</li><li>
Perform special treatments as instructed.</li><li>
Immediately notify the Nurse of any resident leaving/missing from the facility.</li><li>
Use only the equipment you have been trained to use.</li><li>
Report defective equipment to the Nurse Supervisor/Charge Nurse</li><li>
Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.</li><li>
Report all allegations of resident abuse and/or misappropriation of resident property.</li><li>
Honor the resident&rsquo;s refusal of treatment request. Report such requests to your supervisor.</li><li>
Give back rubs as instructed</li><li>
Assist residents with dressing\undressing as necessary</li><li>
Assist residents with hair care functions (i.e., combing, brushing, shampooing, etc.)</li><li>
Assist residents with nail care (i.e., clipping, trimming, and cleaning the finger/toenails)</li><li>
Shave male residents</li><li>
Keep hair on female residents clean (i.e., facial hair, under arms, on legs, etc.) as instructed.</li><li>
Keep residents dry (i.e., changing gown, clothing, linen, etc., when it becomes wet or soiled)</li><li>
Make beds (occupied and unoccupied)</li><li>
Put extra covers on beds as requested</li><li>
Position bedrest residents in correct residents in correct and comfortable positions.</li><li>
Assist residents with bowel and bladder functions (i.e., take to bathroom, offer bed pan, urinal, portable commode, etc.)</li><li>
Maintain intake and output records as instructed.</li><li>
Keep incontinent residents clean and dry.</li><li>
Check and report bowl movements and character of stool as instructed</li><li>
Perform restorative and rehabilitative procedures as instructed.</li><li>
Utilize protective and adaptive equipment as instructed.</li><li>
Provide eye and ear care (i.e., hearing aide installation, cleaning eyeglasses/hearing aids, etc.) as instructed.</li><li>
Assist in preparing the resident for physical examination.</li><li>
Weigh and measure residents as instructed.</li><li>
Measure and record temperatures. Pulse, and respirations (TPRs), as instructed.</li><li>
Ensure that residents who are unable to call for help are checked frequently.</li><li>
Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.</li><li>
Assist with the care of the end-of-life resident.</li><li>
Assist with post-mortem care as instructed.</li></ul>
Supervisory Requirements
This position has no supervisor responsibilities
Qualification
Education and/or Experience
Must possess, as a minimum, a high school education or
its equivalent.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Mathematical Skills
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a
variety of instructions furnished in written, oral, diagram, or schedule form.
Certificates, Licenses, Registrations
To perform this job successfully, an individual must be proficient in the Microsoft Suite products.

General Purpose
The primary purpose of your job position is to assist in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor or charge nurse, and the Health Services Director. The administration of medications shall be in accordance with established nursing standards, the policies, procedures, and practices of this facility, and the requirements of this state.
Essential Duties<ul><li>Follow the policies and procedures of the facility governing the administering of medications to residents.</li><li>Note and report errors in the administration of medications.</li><li>Assist in developing and implementing procedures or programs of the facility that seek to determine what medication errors are taking place, when, where, and why they occur, and how they can be prevented.</li><li>Perform administrative requirements, such as completing necessary forms, charts, reports, etc., and submit these as may be required.</li><li>Maintain a friendly, productive, working relationship with other members of Health Services, attending physicians, the HSD, and pharmacists.</li><li>Report any complaints or grievances made by residents to the Health Services Director.</li><li>Participate and cooperate with any facility surveys (inspections) made by authorized government agencies as requested by the Executive Director or Health Services Director.</li><li>Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process.</li><li>Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the HSD.</li><li>Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or pharmacist.</li><li>Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Health Services Director, including the documentation requirements.</li><li>Follow the labeling policies and practices of the facility.</li><li>Ensure that all medications administered are properly labeled.</li><li>Follow facility procedures in regard to charting medications.</li><li>Assist in documenting and removing medications that are discontinued by the attending physician.</li><li>Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Health Services Director.</li><li>Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you.</li><li>Follow facility procedures in holding medications for residents who are transferred from the facility or when the attending physician discontinues medications</li><li>Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.</li><li>Promptly notify the Health Services Director of any errors or reactions to medications by residents.</li><li>Verify the identity of the resident before administering the medication treatment.</li><li>Accurately measure, record, and report the vital signs of residents.</li><li>Follow the facility&rsquo;s procedures allowing residents to self-administer his or her medications.</li><li>Be familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the facility, pharmacists, and physicians.</li><li>Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your certification on a current status.</li><li>Agree not to disclose resident&rsquo;s protected health information and promptly report suspected or known violations of such disclosure to the Executive Director.</li><li>Report any known or suspected unauthorized attempt to access facility&rsquo;s information system.</li><li>Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors.</li><li>Follow work assignments, and/or work schedules in completing and performing your assigned tasks.</li><li>Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents.</li><li>Maintain the confidentiality of all resident care information including protected health information.
