Job Description Summary</b></h2>
The Clinical Transition Coach (CTC) assists in the safe and timely transition of patients from acute inpatient care to alternative levels of care, including but not limited to a home care program, palliative/hospice program, skilled nursing facility, or home setting appropriate to the patient&rsquo;s needs. The CTC performs activities essential to the discharge of the patient, and may also assist with the admissions process and/or other processes to facilitate patient throughput.<h2 class="jobSectionHeader"><br>

Essential Functions
</b></p><ul><li>Manages and treats patients with a wide-range of acute illnesses, chronic conditions, and diseases in all patients across all age groups, focusing on clinical excellence, patient education, and collaboration.</li><li>
Conducts physical examinations on patients.</li><li>
Orders, performs, and interprets diagnostic and screening tests, such as EKGs, lab work, and imaging studies.</li><li>
Diagnoses, treats, and manages the health care needs of patients; referring to physicians when medically appropriate.</li><li>
Provides age-appropriate preventative health screenings and immunizations (e.g. well-child checks, adult annual physicals, cancer screenings).</li><li>
Consults and collaborates with primary care providers, and other specialists to ensure coordinated and holistic patient care.</li><li>
Performs minor office-based procedures as credentialed</li><li>
Coordinates follow-up care and referrals.</li><li>
Compiles and evaluates medical history, physical findings and diagnostic data to include but not limited to radiographic, laboratory, ultrasound, EKG, EEG, CT and nuclear scans, MRI and MRA studies on all patients to formulate accurate diagnoses.</li><li>
Monitors medications and initiates appropriate therapies and alters medications according to variations in a patient's response within established protocols.</li><li>
Provides and documents patient education regarding treatments, medications, and community resources. Promotes health, disease prevention, and management strategies based on age specific needs.</li><li>
Coordinates the implementation and continuity of medical treatment through activities such as home/hospital findings and other resources.</li><li>
Participates in the orientation and staff development programs for personnel assisting with patient care.</li><li>
