 POSITION SUMMARY:
 
 The Community Health Medical Assistant supports the mission of the Neighborhood Wellness Center by providing high-quality patient care, preventive health screenings, and health education in both clinic and community-based settings. Working under the supervision of the Community Health Nurse and providers, the CMA plays a key role in patient engagement, clinical workflow, and outreach activities designed to address chronic disease prevention and improve health equity in underserved populations.
 
 
 
 Essential Functions
 
 
 
 
 Clinical Support &amp; Screenings:
 
 
 
 <ul>
 <li>
 Perform preventive health screenings (e.g., blood pressure, blood glucose)
 </li>
 <li>
 Collect and record vital signs, and intake information accurately.
 </li>
 <li>
 Support immunization and vaccination workflows under RN/provider supervision.
 </li>
 <li>
 Assist in point-of-care testing (POCT) and specimen collection.
 
 
 </li>
 </ul>
 
 Care Coordination &amp; Outreach
 
 
 
 <ul>
 <li>
 Participate in community-based outreach events (e.g. health fairs, school screenings, church/wellness events).
 </li>
 <li>
 Assist the Community Health RN &amp; CHWs in connecting patients to medical, behavioral health, &amp; social services.
 </li>
 <li>
 Conduct reminder calls and follow-ups for patients needing additional screenings or services.
 
 
 </li>

 Team Collaboration
 
 
 
 <ul>
 <li>
 Work with Community Health RN, CHWs, &amp; providers to ensure smooth clinic flow &amp; comprehensive patient care.
 </li>

 Support interns and volunteers during outreach activities.
 </li>

 Participate in staff meetings, huddles, and quality improvement initiatives.

 Current Medical Assistant certification (CMA) is preferred, but not mandatory

 The Registered Nurse would be a key to providing support for our providers, safe care for our patients, strengthening operational quality, and ultimately improving clinical patient outcomes. This RN assists in the care coordination our patient&rsquo;s healthcare needs, and serves as a teacher, advocate, and navigator for these patients within the context of the care team. This RN will be responsible for providing direct nursing care, performing nursing assessments, participating in the continuity of patient care, being a servant-leader, and helping improve patient outcomes.
 
 
 
 Essential Functions
 
 
 
 <ul>
 <li>
 Provides clinical and administrative support for providers within the care team model. This includes:
 </li>
 <li>
 Communicating abnormal lab results and treatment plan to patients as outlined by the provider and/or protocols
 </li>
 <li>
 Providing patient education in areas such as chronic disease, medications, pregnancy
 </li>
 <li>
 Completing medication reconciliation and new patient intakes.
 </li>
 <li>
 Working with other members of the care team to coordinate patient care, referrals, DME orders, care management and hospital/ER follow-up
 </li>
 <li>
 Executing nursing care under direction of the provider such as: medication injections, wound care and dressing changes, and assisting in procedures
 </li>
 <li>
 Navigating medication changes and prior authorizations due to insurance requirements under standing orders and the guidance of the provider
 </li>
 <li>
 Obtaining pre-authorizations for diagnostic testing as required by insurance
 </li>
 <li>
 Vaccine For Children Lead and handles all mandatory reporting requirements &amp; compliance protocols including excursions when needed
 </li>
 <li>
 Manages the clinical phone line. Assists pharmacies, responds to patient requests, conducts triage and conveys messages to and from providers, clinics, specialists, and other care team members.
 </li>
 <li>
 Directs the assessment of acutely ill patients who present to the clinic in person or via phone to ensure efficient workflow.
 </li>
 <li>
 The position will require management of teams of Medical Assistants and Care Coordinators.

 Participates in care team meetings, monthly nurse meetings, clinic-wide quarterly meetings and other
