Southern Caregivers is a growing In-Home, Personal Care Agency that is looking to add an Intake Nurse to our Jonesboro and Forrest City area. This position is salary-based. Applicants can be LPN or RN. The position will be based in the office along with travel to client's homes throughout the Jonesboro and Forrest City coverage area. Office will be based in Jonesboro and Forrest City. At times, nurse could be asked to help out in other sister territories close by. Nurse would also assist and/or conduct PCA training courses in the office. A work vehicle is provided.RISK OF EXPOSURE TO BLOODBORNE PATHOGENS &ndash; LIMITEDDUTIES OF POSITION: Assists in the coordination of client services under the direction of the Office Manager.POSITION RESPONSIBILITIES:<ul><li>Identifies and evaluates client needs and appropriateness of personal care services.</li><li>Upon receiving the referral for services, the intake nurse should make an initial phone call immediately. If unable to call immediately, an initial call should be made before the end of the same working day.</li><li>Completes intake forms upon client admission. Notify Office Manager and Lead Scheduler of completion of intake (Intake should be completed within 48 hours of receiving referral. Admission paperwork should be completed within 24 hours of intake).</li><li>Communicates on a routine basis with Office Manager, Lead Scheduler, and Scheduling Coordinators in regard to a client&rsquo;s specific needs and/or requests.</li><li>Complete all intakes on Medicaid and/or PASSE clients discharged from hospital or facility within 48hrs of client returning home. Ensure all appropriate supporting documentation for re-assessment is completed and put into client&rsquo;s chart.</li><li>Supervisory Home Visits are to be completed as follows:</li><li>Medicaid/PASSE &ndash; Annually</li><li>AR Choices &ndash; Every 6 months; rotating with aide present and not present.</li><li>VA &ndash; Monthly or as Specified by Authorization</li><li>Complete all PA paperwork for the initial visit as advised by Office Manager. Enter PA request to appropriate portal based on payer source and update Office Manager when request is entered.</li><li>Develop the Care Plan after completing the initial assessment of the client &amp; consulting the client and/or responsible party.</li><li>Documents &amp; reports changes in client status to Office Manager immediately.</li><li>Review and update Care Plans per policy.</li><li>Participates in community related functions per request of the Office Manager and/or marketing staff if schedule allows.</li><li>Assist with PCA courses per request of Office Manager</li><li>The intake nurse must attend and participate in the weekly staff meeting.</li><li>Performs other duties as requested by Office Manager and/or Director</li></ul>QUALIFICATIONS:<ul><li>Currently licensed as an LPN or RN with the State of Arkansas in good standing.</li><li>Has clear criminal background screening.</li><li>Experience in a Home Care setting preferred.</li><li>Must be computer literate and able maintain simple records.</li></ul>JOB CONDITIONS:<ul><li>Position requires in office work 8:00am &ndash; 4:00pm, Monday-Friday</li><li>Position is stressful in terms of meeting deadlines.</li><li>It requires minimal lifting of office records and printouts.</li><li>The ability to read 12 point and larger type is required.</li><li>One must be able to hear adequately on the telephone with no more than an amplifier and be able communicate both verbally and in writing in English.</li><li>Position require travel to and from client homes as well as possible travel to in-service training.</li></ul>EQUIPMENT OPERATION:The job requires the ability to utilize a PC, calculator, multi- line telephone, and other related office equipment.COMPANY INFORMATION:Has access to all patient and financial records, which may be discussed with all managementApplicant must be organized, have good time management, have good people skills, professionalism, able to multi-task, and have good computer skills. Must have a valid driver's license, able to pass a background check and drug screen.
