We're currently looking for an independently licensed mental health professional to join the Care Navigation team (LCSW, LMFT, LPC required) to support members seeking support with posttraumatic stress disorder (PTSD) and complex trauma. Reporting to the Lead Specialty Care Navigator of the Trauma team, the Trauma Clinical Care Navigator will provide initial assessment, support, referral, and ongoing care coordination for members within Spring Health's innovative Trauma program.

This is a full time position that is fully remote. The required shift for this position is Monday-Friday, 9am-5pm PDT (i.e., 12pm-8pm EDT).

What you'll do:
<ul>
<li>Telephonic screening, brief intervention, and referral of members to a trauma specialty treatment provider.</li>
<li>Apply active listening, psychoeducation, and motivational interviewing skills to understand each member's needs and provide support as they set goals.</li>
<li>Utilize Judith Herman's phase based trauma treatment model to provide stabilization support to members in their process of engaging in care.</li>
<li>Utilize motivational interviewing to assist members in engaging in care.</li>
<li>Identify an appropriate level of care, facilitate referral, and coordinate care with a Spring Health provider.</li>
<li>Provide consultation regarding mental health concerns and family/relationship issues, as appropriate to member needs.</li>
<li>As necessary, provide in-the-moment support and safety planning for members with safety, risks or crisis.</li>
<li>Utilize written communication skills to interact with members and providers through email.</li>
<li>Document information, maintain accurate records, perform follow-ups, and track progress within several systems, including an EHR within required timeframes.</li>
<li>Identify and problem-solve issues that serve as a barrier or disruption to care.</li>
<li>Interact with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a smooth member experience.</li>
<li>Assisting other specialty care teams (SUD, Mood and Anxiety, and Eating Disorders), as needed.</li>
<li>Participate in staff meetings, case consultations, and trainings.</li>
<li>Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards.</li>
</ul>
What success looks like:
<ul>
<li>Maintain high member satisfaction, as measured by survey responses</li>
<li>Retain members throughout their course of care by maintaining consistent outreach and contact, as appropriate</li>
<li>Triage and enroll members into appropriate LOC, based on presenting problems and goals</li>
<li>Outreach members via phone and email to schedule triages</li>
</ul>
What you'll bring:
<ul>
<li>Required mental health license: LCSW, LMFT, LPC</li>
<li>Minimum of 2+ years of clinical care experience working with PTSD and Complex Trauma.</li>
<li>Experience with motivational interviewing, diagnostic assessment, and triage.</li>
<li>Knowledge of phased-based trauma treatment model and EBPs for PTSD (e.g. Prolonged Exposure, Cognitive Processing Therapy, EMDR, ART, etc).</li>
<li>Excellent listening, written and verbal communication skills.</li>
<li>Acknowledge and embrace diversity and inclusion.</li>
<li>Experience working with diverse populations.</li>
<li>Motivated to go above and beyond for members to provide a high-touch experience.</li>
<li>Tech-savvy: must be comfortable using various computer platforms and learning new systems, and efficient in tech-related tasks.</li>
<li>Able to multi-task and context switch!</li>
</ul>

The target base salary range for this position is $33.33 - $42.31 Hourly, and is part of a competitive total rewards package including stock options and benefits. Individual pay may vary from the target range and is determined by a number of factors including experience, location, internal pay equity, and other relevant business considerations. We review all employee pay and compensation programs annually using Radford Global Compensation Database at minimum to ensure competitive and fair pay.

This role requires flexibility for occasional shift changes and availability to work holidays that coincide with scheduled shifts.

The required schedule for this role is Saturday-Wednesday: 11am-7pm EST, or the equivalent in your respective time zone (i.e., 10pm-6pm CST).

What you'll be doing:
<ul>
<li>Provide in-the-moment clinical support, psycho education, and connection to appropriate referrals and resources on a clinical phone queue.</li>
<li>Utilize skills such as active listening, motivational interviewing, and solution-focused approaches to collaboratively engage with members seeking help with mental health, substance use, or family/relationship concerns.</li>
<li>Assess for risks and provide crisis support, safety planning, and follow up with high risk members to ensure safety and timely connection to care.
</li><li>Recommend a plan of action based on clinical presentation and coordinate connection to appropriate care and services (Spring Health therapists, coaches, physicians, or external resources).</li>
<li>Provide after hours support to members and leaders seeking services or connection to the Global team, Management Consultation team, and Spring Internal team.</li>
<li>Use effective written skills to complete professional documentation and to interact with members and providers through email.</li>
<li>Follow care management outreach protocols and utilize good clinical judgment to identify and problem-solve barriers or disruptions to care.</li>
<li>Promote effective care by coordinating closely with Spring Health care providers and the Provider Operations team.</li>
<li>Interact and problem-solve with multi-disciplinary teams such as Care Support, Benefits/Billing, and Product to ensure a smooth member experience.</li>
<li>Participate in staff meetings, case consultations, and trainings.</li>
<li>Assist with onboarding and mentoring new team members.</li>
<li>Follow Spring Health policies and maintain all confidentiality, compliance, and ethical standards.</li>

What we expect from you:

<li>Minimum of 2+ years of direct clinical care experience.</li>

<li>Fully engaged and free from interruptions for the duration of shift.</li>
